目的 :研究下肢深静脉瓣膜关闭功能不全的分型与影像学诊断。材料与方法 :2 16条肢体均行下肢深静脉顺行造影 ,其中 15 2条肢体行经皮月国静脉插管造影 ,并对 14条单纯性大隐静脉曲张 (正常瓣膜 )肢体进行血液动力学观察。结果 :2 0 2条肢体诊断为下肢静脉关闭下全 ,14条肢体诊断为单纯型大隐静脉曲张 ,其中 ,Ⅰ型 :深静脉瓣膜脱垂致瓣膜关闭不全 2 6例 ;Ⅱ型 :深浅静脉扩张致瓣膜相对关闭不全 3 3例 ;Ⅲ型 :瓣膜脱垂加股静脉扩张 3 6例 ;Ⅳ型 :先天性下肢深静瓣膜发育不全症 4例 ;Ⅴ型 :深静脉血栓形成后完全再通瓣膜遭破坏 3 9例。 14条单纯性大隐静脉 ,瓣膜完全正常 ,屏气时正常肢体股静脉第一对瓣膜、月国静脉瓣膜同时关闭 ,足背伸屈运动时月国静脉瓣膜开放程度显著大于股静脉第一对瓣膜 (P <0 0 1)。结论 :下肢深静脉瓣膜关闭功能不全能全面反映疾病的病理本质 ,较下肢深静脉瓣膜功能不全的概念更确切
Objective: To study the classification and imaging diagnosis of deep venous valve insufficiency of lower extremity. MATERIALS AND METHODS: Twenty-two limbs underwent deep venous angiography of lower extremity. 15 2 limbs were percutaneously injected through the celiac vein and the hemodynamics of 14 limbs of simple saphenous vein (normal valve) Observed. Results: 220 limbs were diagnosed as complete lower extremity vein closure and 14 limbs were diagnosed as simple saphenous varicosities. Among them, type Ⅰ: 26 cases with valvular insufficiency due to deep venous valve prolapse; type Ⅱ: deep and superficial veins Type 3: valvular prolapse plus femoral vein expansion in 36 cases; Type IV: congenital lower limb deep valvular hypoplasia in 4 cases; type Ⅴ: deep vein thrombosis after complete recanalization Valvular destruction 39 cases. 14 simple saphenous vein, the valve is completely normal, hold the air when the first normal limb femoral vein valve, the country vein valve closed at the same time, dorsal flexion and extension movement of the country when the valve is significantly greater than the valve opening of the first pair of femoral vein (P <0 0 1). CONCLUSIONS: The closed dysfunction of deep venous valve of the lower extremity can fully reflect the pathological nature of the disease, which is more exact than the concept of deep venous insufficiency of the lower extremity
