目的分析C肽、胰岛素及皮质醇与新生儿低血糖间的相关性。方法选取2015年1月-2017年1月本院新生儿科收治的新生儿低血糖258例为研究对象,另取正常血糖新生儿80例为对照,对比低血糖组与正常血糖组C肽、胰岛素及皮质醇水平,分析低血糖组C肽、胰岛素及皮质醇水平与低血糖持续时间及低血糖数值间的相关性。结果低血糖组新生儿的C肽、胰岛素水平高于正常组,皮质醇水平则明显低于正常组,差异均具有统计学意义(P<0.05)。C肽、胰岛素与低血糖持续时间呈正相关(r=0.748,0.814,P均<0.001);皮质醇水平与低血糖持续时间呈负相关(r=-0.809,P<0.001)。C肽、胰岛素与低血糖数值呈正相关(r=0.832,0.786,P均<0.001);皮质醇水平与低血糖数值呈负相关(r=-0.623,P<0.001)。结论 C肽、胰岛素、皮质醇与新生儿低血糖的发生存在相关性;对于C肽、胰岛素与皮质醇含量异常的新生儿,即使血糖检测正常,也应注意低血糖的发生,减少漏诊,及时干预,可以减少新生儿低血糖后遗症的发生。
Objective To analyze the correlation between C-peptide, insulin and cortisol and neonatal hypoglycemia. Methods From January 2015 to January 2017, 258 neonates with hypoglycemia admitted to neonatology department of our hospital were selected as the research object. Another 80 normal newborns were used as control. The levels of C-peptide, insulin, And cortisol levels, the correlation between hypoglycemic C-peptide, insulin and cortisol levels and hypoglycemic duration and hypoglycemic values was analyzed. Results The levels of C-peptide and insulin in newborn infants with hypoglycemia were significantly higher than those in normal controls, and the levels of cortisol in neonates with hypoglycemia were significantly lower than those in normal controls (P <0.05). C peptide, insulin and duration of hypoglycemia were positively correlated (r = 0.748,0.814, P <0.001). Cortisol level was negatively correlated with duration of hypoglycemia (r = -0.809, P <0.001). C peptide, insulin and hypoglycemic values were positively correlated (r = 0.832, 0.786, P <0.001). Cortisol level was negatively correlated with hypoglycemia (r = -0.623, P <0.001). Conclusion C-peptide, insulin and cortisol are correlated with neonatal hypoglycemia. For newborns with C-peptide, insulin and cortisol abnormalities, we should pay attention to the occurrence of hypoglycemia, reduce the missed diagnosis and timely Intervention can reduce neonatal hypoglycemic sequelae.
