目的 :探讨完全埋伏阻生第三磨牙拔除术中、术后并发症、预防措施和解决方法。方法 :通过临床检查和曲面体层片确定376颗下颌第三磨牙完全埋伏阻生,术中采用常规Ward切口和改良Ward切口翻瓣,凿骨劈冠和高速涡轮机切割牙体,术后常规给予口服抗感染、止痛、消肿药物,统计12~25岁、26~40岁、40岁以上3个不同年龄段发生各种并发症的例数。采用SPSS 13.0软件包对数据进行统计学分析。结果:12~25岁、26~40岁、40岁以上3个不同年龄段下颌埋伏阻生第三磨牙拔除术中、术后并发症,包括软组织损伤、术后出血、面部肿胀、疼痛、开口受限无显著差异(P>0.05),邻牙损伤、舌侧骨板骨折、颞下颌关节区疼痛、干槽症、舌及下唇感觉障碍有统计学差异(P<0.05)。结论 :邻牙损伤、舌侧骨板骨折、颞下颌关节区疼痛、干槽症、舌及下唇感觉障碍等并发症与操作者技术水平、患者年龄段有密切关系,早期拔除可预防及减少并发症的发生。
Objective: To investigate the complications, precautionary measures and solutions of the third molars with completely buried obstruction. METHODS: A total of 376 mandibular third molars were confirmed by clinical examination and surface slice. All patients were treated with conventional Ward incision, modified Ward incision flap, chisel bite crown and high-speed turbine. The patients were routinely given postoperative Oral anti-infective, analgesic, anti-swelling drugs, statistics 12 to 25 years old, 26 to 40 years old, 40 years of age at three different age groups occurred a variety of complications. Data were statistically analyzed using SPSS 13.0 software package. Results: Complications such as soft tissue injury, postoperative bleeding, facial swelling, pain and opening were found in the third molars with impacted mandibular implants at 12 ~ 25 years old, 26 ~ 40 years old and over 40 years old. There was no significant difference between the two groups (P> 0.05). There was significant difference between adjacent tooth injury, lingual fracture, temporomandibular joint pain, dry socket, tongue and lower lip sensation (P <0.05). Conclusion: Complications of orthodontic tooth injury, lingual fracture of bone, temporomandibular joint area, dry socket, tongue and lower lip sensation are closely related to the technical level of the operator and the patient’s age. Early removal can prevent and reduce Complications occur.
