目的了解宁波地区60岁及以上老年人群糖尿病患病情况,为糖尿病的防治工作提供科学依据。方法采用随机整群抽样方法,对60岁及以上老年人群进行糖尿病筛查。共调查老年人群5 081人,男性2 173人(42.77%),女性2 908人(57.23%);年龄60~94岁,平均(68.1±6.9)岁。结果糖尿病检出率为9.29%,其中男性糖尿病检出率为8.65%,女性检出率为9.77%,性别间差异无统计学意义(χ2=1.83,P>0.05);糖尿病检出率随年龄增长而增加(趋势χ2=51.28,P<0.01)。糖尿病前期检出率为25.49%,男性检出率为24.02%,女性检出率为26.58%,男女性别间糖尿病前期检出率差异无统计学意义(χ2=4.29,P>0.05);糖尿病前期检出率随年龄增长而略有升高(趋势χ2=5.42,P<0.05)。鄞州区西片(山区)老年人群糖尿病和糖尿病前期的检出率均明显低于鄞州区东片和中片(平原),差异有统计学意义(P<0.01)。现场筛查中,仅做空腹血糖则糖尿病漏检率达58.47%;若仅做糖耐量试验则糖尿病漏检率为13.14%。结论糖尿病是60岁及以上老年人群严重的公共卫生问题,空腹血糖检测和糖耐量试验2种方法联合应用在糖尿病的现场筛查工作中具有重要的作用。
Objective To understand the prevalence of diabetes in the elderly population aged 60 years and above in Ningbo and to provide a scientific basis for the prevention and treatment of diabetes. Methods A randomized cluster sampling method was used to screen diabetes patients aged above 60 years old and above. A total of 5 081 elderly people were surveyed, 2 173 (42.77%) were men and 2 908 (57.23%) were women, aged 60-94 years (mean, 68.1 ± 6.9) years. Results The detection rate of diabetes was 9.29%. The detection rate of male diabetes was 8.65% and that of female was 9.77%. There was no significant difference between sexes (χ2 = 1.83, P> 0.05). The detection rate of diabetes with age Increasing (χ2 = 51.28 trend, P <0.01). Pre-diabetes detection rate was 25.49%, male detection rate was 24.02%, female detection rate was 26.58%, male and female gender differences in pre-diabetes detection rate was not statistically significant (χ2 = 4.29, P> 0.05); pre-diabetes The detection rate increased slightly with age (trend χ2 = 5.42, P <0.05). The prevalence of diabetes mellitus and pre-diabetes in Yinzhou District western film (mountainous area) was significantly lower than that in the east and middle of Yinzhou District (P <0.01). On-the-spot screening, only the fasting blood glucose, diabetes missed the rate of 58.47%; if only glucose tolerance test diabetes missed the rate of 13.14%. Conclusion Diabetes mellitus is a serious public health problem in the elderly population aged 60 and above. The combined application of fasting blood glucose test and glucose tolerance test plays an important role in the field screening of diabetes mellitus.
