目的探讨人脑胶质瘤组织中Ki-67和基质金属蛋白酶(matrix metalloproteinase,MMP)-9的表达水平及与肿瘤恶性程度的关系。方法收集2004年6月—2014年5月泰安市中心医院神经外科脑胶质瘤手术切除标本,分为低度恶性组(30例)、高度恶性组(30例)、复发组(15例),另取颅脑外伤手术中切除的破碎脑组织标本15例作为创伤破碎脑组织组,应用免疫组织化学技术检测75例不同病理级别胶质瘤组织及15例颅脑外伤手术中切除的破碎脑组织中Ki-67和MMP-9的表达情况,并对脑胶质瘤病理级别进行相关分析。计量资料采用t检验,相关性检验用Pearson相关分析,P<0.05为差异有统计学意义。结果创伤破碎脑组织组、低度恶性组、高度恶性组、复发组Ki-67阳性表达百分比分别为(0.000±0.000)%、(15.370±5.176)%、(26.130±4.624)%、(29.520±4.441)%,创伤破碎脑组织组与其余各组比较,差异均有统计学意义(均P<0.05);低度恶性组与高度恶性组和复发组比较,差异均有统计学意义(均P<0.05),低度恶性组与复发组比,差异均有统计学意义(P<0.05)。创伤破碎脑组织组、低度恶性组、高度恶性组、复发组MMP-9阳性表达百分比分别为(1.245±0.390)%、(28.030±3.943)%、(39.320±3.987)%、(41.230±5.377)%,创伤破碎脑组织组与其余各组比较,差异均有统计学意义(均P<0.05),低度恶性组与高度恶性组、复发组相比较,差异均有统计学意义(均P<0.05),高度恶性组与复发组比较,差异无统计学意义(P>0.05)。不同组别脑胶质瘤中Ki-67表达的阳性细胞百分比与MMP-9表达的阳性细胞百分比呈正相关性,差异有统计学意义(P<0.05)。结论脑胶质瘤中Ki-67和MMP-9表达水平可用来评价肿瘤的恶性程度,其中Ki-67表达水平可为术后复发率提供必要的参考依据。
Objective To investigate the expression of Ki-67 and matrix metalloproteinase (MMP-9) in human glioma and its relationship with malignancy of tumor. Methods The surgical specimens of neurosurgery for glioma from June 2004 to May 2014 in Tai’an Central Hospital were collected and divided into three groups: low grade group (30 cases), high grade group (30 cases), recurrent group (n = 15) , And another 15 cases of broken brain tissue removed during craniocerebral trauma were used as traumatic brain tissue. Immunohistochemistry was used to detect 75 cases of glioma of different pathological grades and 15 cases of traumatic brain resection in traumatic brain injury The expression of Ki-67 and MMP-9 in glioma tissue were analyzed. Measurement data using t test, correlation test using Pearson correlation analysis, P <0.05 for the difference was statistically significant. Results The percentage of positive expression of Ki-67 in traumatic brain injury group, low grade group, high grade group and recurrent group were (0.000 ± 0.000)%, (15.370 ± 5.176)%, (26.130 ± 4.624)%, (29.520 ± 4.441)% respectively. There was significant difference between the traumatic brain injury group and the other groups (all P <0.05); the difference between the low-grade malignant group and the highly malignant group and the recurrent group was statistically significant (P <0.05). The positive percentage of Ki-67 in different groups of gliomas was positively correlated with the percentage of positive cells expressed in MMP-9, with statistical significance (P <0.05). Conclusion The expression of Ki-67 and MMP-9 in glioma can be used to evaluate the malignant degree of tumor. Ki-67 expression level may provide necessary reference for the postoperative recurrence rate.
