目的 探讨组织谐波成像技术 (tissue harmonic imaging,THI)在肾上腺占位性病变诊断中的应用价值 ,旨在提高肾上腺肿瘤的超声诊断率。方法 应用常规二维超声 (two- dimensional ultrasound,2 D)与 THI对肾上腺肿瘤进行对比观察。 6 7例经手术病理证实的肾上腺肿瘤 ,按肿块大小分为直径≤ 3cm (组 1)及 >3cm(组 2 )两组。对比分析肿瘤的 2 D和 THI的诊断符合率 ,并将 2 D及 THI的肿瘤图像按显示清晰与不清晰进行分级比较。结果 组 1,2 D及 THI诊断符合率分别为 5 2 .2 %及 75 .9% ,二者比较差异有显著意义 (P<0 .0 5 )。组2 ,2 D及 THI诊断符合率均为 92 .1% ,二者比较无差异 (P>0 .0 5 )。两组肿块 2 D及 THI图像显示清晰率分别为 34.5 %和 82 .6 %及 81.6 %和 92 .1% ,二者比较差异均有显著意义 (P<0 .0 5 )。结论 本研究表明 ,THI可提高肾上腺肿瘤的图像清晰度及对比度 ,尤其是改善小肿块的图像质量 ,从而提高诊断率。
Objective To investigate the value of tissue harmonic imaging (THI) in the diagnosis of adrenal space-occupying lesions in order to improve the diagnostic rate of adrenal tumors. Methods Two-dimensional ultrasound (2-D) and THI were used to observe the adrenal tumors. Sixty-seven cases of adrenal tumors confirmed by surgery and pathology were divided into two groups according to the size of the tumor, the diameter ≤ 3cm (group 1) and> 3cm (group 2). The diagnostic coincidence rates of 2-D and THI were compared between the two groups. The tumor images of 2-D and THI were classified and compared clearly and clearly. Results The coincidence rates of diagnosis of group 1, 2 D and THI were 52.2% and 75.9%, respectively, with significant difference (P <0.05). The coincidence rate of diagnosis of group 2, 2 D and THI was 92.1%, there was no difference between the two groups (P> 0.05). The 2D and THI images of the two groups showed 34.5% and 82.6% and 81.6% and 92.1%, respectively, with significant difference (P <0. 05). Conclusion This study shows that THI can improve the image resolution and contrast of adrenal tumors, especially improving the image quality of small lumps, thus improving the diagnosis rate.
