目的:通过研究外周血单个核细胞早期凋亡率、fas系统基因及其蛋白表达水平,考察外周血单个核细胞(peripheral blood mononuclear cells,PBMCs)早期凋亡率、mFas、mFasL、sFas和sFasL分子与SLE患者疾病活动度之间的关系。方法:病例选自两所三甲医院住院SLE患者,以健康志愿者为对照。利用流式细胞仪,采用连接素V和碘化丙啶双染法检测研究对象的PBMCs早期凋亡率及PBMCs膜上mFas和mFasL的表达水平;以ELISA测定研究对象外周血sFas和sFasL分子的表达水平。结果:SLE患者的PBMCs早期凋亡率(13.29%±4.12%,n=76)高于正常对照组(4.66%±1.57%,n=69)(Z=9.657,P<0.01),活动期患者(SLEDAI≥10)的早期凋亡率(14.80%±5.09%,n=53)高于非活动期(SLEDAI<10)(9.82%±3.76%,n=23),高于正常对照(4.66%±1.57%,n=69)(F=120.28,P<0.01)。多重线性回归分析结果表明,对于SLE患者和非活动期SLE患者,最终进入方程的自变量仅为PBMCs早期凋亡率(apoptosis)。对于活动期SLE患者,最终进入方程的自变量有PBMCs早期凋亡率(apoptosis)、血清Fas(sFas)两项。结论:SLE患者的PBMCs早期凋亡率显著升高,且与SLE患者疾病活动性指数的关系最为密切,临床上可作为SLE患者疾病活动度的监测参考指标之一;而对于活动期SLE患者,血清Fas(sFas)也能较好的反映患者疾病活动度。
