目的观察组织胶注射联合套扎治疗食管胃底静脉曲张的疗效及安全性。方法对37例食管胃底静脉曲张破裂出血患者进行内镜下组织胶注射,对合并的食管曲张静脉同时行套扎治疗,观察近期疗效、不良反应和并发症。结果组织黏合剂注射量1.0～3.2ml,平均1.9ml;套扎共行68例次,每例1～3次,平均1.4次,套扎217处。治疗成功率100.0%。9例(24.3%)出现胸痛,3例(8.1%)出现上腹不适,未有异位栓塞发生,随访3个月未有再发出血患者。31例3个月后复查胃镜,见胃底曲张静脉消失或明显减轻,食管静脉曲张减轻及红色征消失。结论组织胶注射联合套扎治疗食管胃底静脉曲张破裂出血安全、有效。
Objective To observe the efficacy and safety of tissue glue injection combined with ligation in the treatment of esophageal varices. Methods Thirty-seven patients with esophageal variceal bleeding underwent endoscopic tissue glue injection. The combined esophageal varices were simultaneously ligated. The curative effect, side effects and complications were observed. Results The amount of tissue adhesive injection was 1.0-3.2ml, with an average of 1.9ml. There were 68 cases of ligation in total, 1.4 times per each case, with an average of 217 ligation. The success rate of treatment was 100.0%. Chest pain was found in 9 cases (24.3%), upper abdominal discomfort in 3 cases (8.1%), no ectopic embolization occurred, and no recurrence of bleeding occurred within 3 months of follow-up. Gastroscopy was performed in 31 patients 3 months later. The varices disappeared or conspicuously disappeared in the fundus. The esophageal varices were relieved and the red signs disappeared. Conclusion Tissue injection combined with ligation in the treatment of esophageal and gastric variceal bleeding is safe and effective.
