磷酸肌酸钠治疗儿童病毒性心肌炎的临床疗效及其作用机制

目的分析磷酸肌酸钠(CPS)治疗儿童病毒性心肌炎(VMC)的临床疗效及其作用机制。方法选取2014年12月—2016年12月湖北医药学院附属随州医院儿科收治的VMC患儿80例,随机分为对照组和观察组,每组40例。在常规治疗基础上,对照组患儿给予注射用果糖二磷酸钠,而观察组患儿给予注射用CPS;两组患儿均连续治疗2周。比较两组患儿临床疗效,治疗后心肌酶谱指标[包括肌酸激酶同工酶(CK-MB)、α-羟丁酸脱氢酶(α-HBDH)、心肌肌钙蛋白I(cTnⅠ)]、抗氧自由基指标[包括超氧化物歧化酶(SOD)、晚期蛋白氧化产物(AOPP)、丙二醛(MDA)]、心脏收缩功能指标[射血分数(EF)、心室短轴缩短率(FS)]。结果观察组患儿临床治疗有效率高于对照组(P<0.05)。治疗后观察组患儿CK-MB、α-HBDH、cTnⅠ及AOPP低于对照组,SOD、MDA及EF、FS高于对照组(P<0.05)。结论 CPS治疗儿童VMC的临床疗效确切,可有效控制患儿病情,保护患儿心功能,考虑主要与其抗氧自由基作用有关。 Objective To analyze the clinical efficacy and mechanism of sodium creatine phosphate (CPS) in the treatment of children with viral myocarditis (VMC). Methods From December 2014 to December 2016, 80 children with VMC admitted to Suizhou Hospital affiliated to Hubei Medical College were randomly divided into control group and observation group, 40 cases in each group. On the basis of routine treatment, the control group of children given injection of sodium fructose diphosphate, while the observation group given CPS injection; two groups of children were treated for 2 weeks. The clinical efficacy of the two groups were compared. After treatment, the indexes of myocardial enzymes [including CK-MB, α-HBDH, cTnⅠ, ], Indicators of antioxidant free radicals [including SOD, AOPP, MDA], systolic function index [EF], shortening of ventricular short axis Rate (FS)]. Results The effective rate of clinical treatment in observation group was higher than that in control group (P <0.05). After treatment, the levels of CK-MB, α-HBDH, cTnⅠ and AOPP in the observation group were lower than those in the control group, while the levels of SOD, MDA, EF and FS in the observation group were higher than those in the control group (P <0.05). Conclusion The clinical efficacy of CPS in the treatment of children with VMC is accurate, which can effectively control the condition of children and protect the heart function of children. It is mainly concerned with the effect of anti-oxygen free radical.
