随着SPECT的引入,肝脏闪烁显像重新具有重要价值。作者在肝脏闪烁检查时使用常规SPECT横断面来计算肝脏和脾脏的体积,并与该病人TCT检查结果相比较。方法:预先使用置满水的人体无病变模型中的肝脏模型进行体外测定。肝脏模型中逐次加入含有0.4mCi~(99m)Tc-DTPA的0.8%盐溶液200ml,使其体积自200ml起以9个相同的等级递增至2000ml。用备有高分辨力平行孔准直器的常规γ照相机取得SPECT影像。置20%中心窗于140keV。数据按64等分角间隔储存。每次检查约1千万计数,故每一影像计数大于15万。γ照相机接上微型计算机,用Karolinska学院的软件重建断层影像。用算术平均法校正影象的衰减。使用Shepp-logan滤器进行影像重建。横断面理论上的厚度为0.625cm,影像矩阵64×64。总和整个器官所有象素的计数以取得器官体积外形轮廓的比例。以体外试验导出的回归系数作为计算病人肝脏和脾脏体积的基础。SPECT体积测定的结果与TCT测定结果进行比较。经过上述预试验后,用SPECT和TCT两种方法检查了50例患者。静脉注射4mCi~(99m)Tc-植酸盐
With the introduction of SPECT, scintigraphy of the liver is once again of great value. The authors used conventional SPECT cross-sections to calculate liver and spleen volumes at liver scintigraphy and compared them to the TCT results of this patient. Methods: In vitro assays were performed in advance using a liver model in a human-filled, non-diseased human model. In a liver model, 200 ml of a 0.8% saline solution containing 0.4 mCi to 99 mTc-DTPA was sequentially added to increase the volume of the liver model to 2000 ml at 9 equal levels from 200 ml. SPECT images were taken with a conventional gamma camera equipped with a high-resolution parallel aperture collimator. Set 20% center window at 140keV. The data is stored in 64 equal angular intervals. Each inspection of about 10 million counts, so each image counts more than 150,000. The gamma camera was connected to a microcomputer and the tomographic images were reconstructed using Karolinska Institute software. Arithmetic averaging to correct the image attenuation. Image reconstruction using a Shepp-logan filter. The theoretical thickness of the cross section is 0.625 cm and the image matrix is 64 × 64. The sum of the counts of all pixels of the entire organ is taken as the ratio of the outline of the organ volume. The regression coefficients derived from in vitro tests serve as the basis for calculating the volume of liver and spleen in patients.
