目的:探讨益心解毒方对心梗后心衰大鼠损伤心肌的保护作用机制。方法:利用左冠状动脉结扎术制备大鼠心梗后心衰动物模型,通过宏观表征结合理化指标综合判定该疾病模型的证候属性,并给予对证中药的干预。检测各组大鼠的心脏超声指标;通过血生化的方法检测血清中肌酸激酶(CK)的水平以及利用酶联免疫的方法测定血清中肌钙蛋白I(CTNI)的水平;并通过放免的方法检测血浆中内皮素(ET)与降钙素基因相关肽(CGRP)的浓度。结果:术后21d,模型组大鼠的心功能显著下降,主要表现在左室舒张末内径(LVEDd)、左室收缩末内径(LVEDs)明显增大,短轴缩短率(FS)以及射血分数(EF)急剧下降(P<0.01);血清中CK、CTNI水平显著升高(P<0.01);血浆中ET水平升高(P<0.01),CGRP水平下降(P<0.01)。给予益心解毒方干预后,能够有效的降低心衰大鼠扩张的LVEDd和LVEDs(分别下降19%和25%,P<0.05和P<0.01),尤以改善LVEDs为主(P<0.01),提高EF(提高25%,P<0.01);并且降低心衰大鼠血浆中上升的ET浓度(P<0.05),升高血浆中CGRP浓度(P<0.01),且进一步降低ET/CGRP的比值(P<0.01);显著下调心衰大鼠的血清中CK、CTNI(P<0.01,P<0.05)。结论:益心解毒方能够有效的提高心衰大鼠的心功能,其机制与降低血中ET、CK、CTNI,并升高血中的CGRP水平等心肌保护作用有关。
Objective: To investigate the protective effect of Yixinjiedu Decoction on myocardial injury in heart failure rats after myocardial infarction. Methods: The animal model of heart failure after myocardial infarction was prepared by ligation of the left coronary artery. The syndromes of the disease model were comprehensively determined by macroscopic characterization and physical and chemical indexes, and the intervention of traditional Chinese medicine was given. The indexes of serum creatine kinase (CK) were detected by blood biochemical method and the level of serum troponin I (CTNI) was measured by enzyme-linked immunosorbent assay. Methods The concentration of endothelin (ET) and calcitonin gene related peptide (CGRP) in plasma were measured. Results: At the 21st day after operation, the cardiac function of the model group decreased significantly, mainly in left ventricular end-diastolic diameter (LVEDd), left ventricular end-systolic diameter (LVEDs), shortening of short axis (FS) and ejection The levels of CK and CTNI in serum were significantly increased (P <0.01). The levels of ET in plasma increased (P <0.01) and the levels of CGRP decreased (P <0.01). The intervention of Yixinjiedu decoction could effectively reduce the LVEDd and LVEDs of dilated rats (19% and 25%, P <0.05 and P <0.01, respectively), especially the improvement of LVEDs (P <0.01) , Increased EF (25%, P <0.01), decreased plasma ET level (P <0.05), increased plasma CGRP level (P <0.01), and further decreased ET / CGRP (P <0.01). The levels of serum CK and CTNI in heart failure rats were significantly decreased (P <0.01, P <0.05). Conclusion: Yixin Jiedu decoction can effectively improve heart function in heart failure rats. The mechanism of Yixin Jiedu decoction is related to decreasing myocardial ET, CK, CTNI and increasing CGRP level in blood.
