目的 探讨彩色多普勒能量图 (CDPI)血流检测及血管内皮生长因子 (VEGF)表达对乳腺肿块中血管生成的判定作用以及二者的相关关系。方法 对 1 0 3例乳腺肿块术前行CDPI血流检测 ,术后标本行免疫组化法检测VEGF的表达。结果 CDPI血流检测乳腺肿块的阳性率为 83 .5 % (86/ 1 0 3) ,其中乳腺癌检出阳性率为 1 0 0 % (66/ 66)远远高于良性肿块的阳性率 54 .1 % (2 0 / 37) (P <0 .0 0 5) ;VEGF在乳腺癌的表达阳性率 1 0 0 % (66/ 66)远远高于良性肿块的表达阳性率 8.1 % (3/ 37) (P <0 .0 0 5) ,有淋巴结转移组表达强度明显高于无淋巴结转移组 (P <0 .0 0 5)。乳腺癌中CDPI血流检出率与VEGF表达阳性率呈正相关关系 (P <0 .0 0 5)。结论 血管形成与乳腺癌的生长、浸润和转移有密切关系 ,CDPI血流检测对于乳腺肿块良恶性的鉴别有重大意义 ,而且CDPI与VEGF的联合检测为判定肿瘤内血管形成水平的有效指标之一。
Objective To explore the role of color Doppler flowmetry (CDPI) in the detection of blood flow and the expression of vascular endothelial growth factor (VEGF) on angiogenesis in breast tumor and the correlation between the two. Methods One hundred and three cases of breast masses were examined with CDPI flow before operation. The expression of VEGF was detected by immunohistochemistry after operation. Results The positive rate of CDPI was 83.5% (86/103). The positive rate of breast cancer was 100% (66/66), which was much higher than the positive rate of benign mass . The positive rate of VEGF expression in breast cancer was 100% (66/66), which was significantly higher than that of benign tumor (8.1%, 3%, P <0.05) / 37) (P <0.05). The expression intensity of lymph node metastasis group was significantly higher than that of no lymph node metastasis group (P <0.05). There was a positive correlation between the detection rate of CDPI and the positive rate of VEGF in breast cancer (P <0.05). Conclusion The formation of blood vessels is closely related to the growth, invasion and metastasis of breast cancer. The detection of CDPI blood flow is of great significance for the identification of benign and malignant breast masses. Combined detection of CDPI and VEGF is one of the effective indicators to determine the level of intravascular angiogenesis .
