目的 探讨亚低温和 (或 )非氨酯对缺氧缺血 (hypoxia ischemia ,HI)脑损伤的保护作用。方法 将HI模型鼠 (右颈总动脉结扎后给予 8%氧气 2h)随机分为亚低温组 (脑温降低 4 5℃ ) :包括亚低温 2 4h组 (n =2 6 )、亚低温 72h组 (n =48)、非氨酯组 (n =48)、亚低温 2 4h与非氨酯联合治疗组(n =5 1)和安慰剂组 (n =48)。对照组 (n =48)为假手术动物。监测动物的肛温和脑温。以血浆S 10 0蛋白 (S 10 0 )和肌酸激酶脑型同功酶 (CK BB)水平、病理检查脑损伤分数 (BIS)、脑海马神经元死亡率 (HDNP)和水迷宫实验结果判断疗效。结果 脑温低于肛温 0 3~ 0 5℃ (r =0 99,P <0 0 1) ,亚低温 2 4h组、亚低温 72h组、联合组动物肛温降低了 4~ 5℃ ,非氨酯组与安慰剂组肛温维持在 36~37℃之间。安慰剂组血浆S 10 0、CK BB水平于HI后 2 4~ 48h达峰值 (分别为 1 2 1~ 1 2 4μg/L和 5 2 9~ 5 4 2IU/L ) ,为对照组的 2~ 3倍 ,各干预组的S 10 0和CK BB水平明显低于安慰剂组 (P <0 0 1)。HI后 72h、14d、6 0d亚低温 72h组BIS和HDNP降低程度为 5 8%、85 %、87% ,联合组为 5 6 %、77%、76 % ,亚低温 2 4h组为 35 %、5 8%、47% ,非氨酯组为 35 %、6 6 %、6 7%。空间记忆功能依次排列为对照组 >联合组 >亚低温 72h?
