目的探讨三维彩色血管能量成像(3D-CPA)联合术后肿瘤标本组织内的微血管密度(micro-vasculardensi-ty,MVD)测量对鉴别卵巢肿瘤良恶性的临床价值。方法2003年12月至2005年5月对中国医科大学附属盛京医院43例临床怀疑为卵巢肿瘤的患者进行3D-CPA,术后应用抗血管内皮生长因子CD34抗体,利用免疫组化SP染色法对病理组织切片进行免疫组织化学染色,计数肿瘤的MVD。结果在良恶卵巢肿瘤中MVD的表达有明显的差异性,恶性肿瘤的MVD定量表达明显高于良性肿瘤(P<0.001),3D-CPA在鉴别诊断恶性卵巢肿瘤方面有较高的特异性,与病理诊断结果的符合率(准确性)可达81.4%,但却有比较高的假阳性率。结论3D-CPA对于恶性卵巢肿瘤的定性诊断有很高的特异性。在鉴别诊断卵巢肿瘤良恶性方面3D-CPA与MVD具有良好的一致性,可以作为术前检查应用于卵巢肿瘤良恶性的诊断。
Objective To investigate the clinical value of 3D-CPA combined with postoperative micro-vascular densitometry (MVD) in differential diagnosis of benign and malignant ovarian tumors. Methods From December 2003 to May 2005, 3D-CPA was performed on 43 cases of clinically suspected ovarian tumors in Shengjing Hospital affiliated to China Medical University. Anti-VEGF-CD34 antibody was used postoperatively. SP immunohistochemical staining Histopathological sections were immunohistochemically stained for tumor MVD. Results The expression of MVD was significantly different between benign and malignant ovarian tumors. The quantitative expression of MVD in malignant tumors was significantly higher than that in benign tumors (P <0.001). 3D-CPA was highly specific in the differential diagnosis of malignant ovarian tumors, The coincidence rate (accuracy) with pathological diagnosis up to 81.4%, but there is a higher false positive rate. Conclusion 3D-CPA has a high specificity for the qualitative diagnosis of malignant ovarian tumors. In the differential diagnosis of benign and malignant ovarian tumors, 3D-CPA has good consistency with MVD and can be used as a preoperative examination in the diagnosis of benign and malignant ovarian tumors.
