目的探讨复合保温模式对体外膜肺氧合辅助肺移植术中体温的影响。方法将40例体外膜肺氧合辅助肺移植术患者随机分为实验组和对照组两组,每组20例,所有患者均采用脉搏指示连续心排血量监测(Pi CCO)记录体核温度,观察两组患者在手术过程中体温易于降低的四个阶段,即全麻开始阶段、ECMO转流开始阶段、供肺移植阶段、移植肺开放阶段的体温变化。结果比较两组患者在全麻开始阶段、ECMO转流开始阶段、供肺移植阶段、移植肺开放阶段,Pi CCO监护仪上的体核温度数据,差异均有统计学意义(P<0.05),实验组患者体温均保持在36℃以上。结论复合保温模式在ECMO辅助肺移植术中能有效维持患者体温稳定在正常范围之内,对手术室护理工作具有指导意义,保障了ECMO辅助肺移植术的安全性。
Objective To investigate the effect of composite thermal insulation on body temperature during lung oxygenation assisted lung transplantation. Methods Forty patients with extracorporeal membrane oxygenation assisted lung transplantation were randomly divided into two groups (experimental group and control group, 20 cases in each group). All patients were recorded with PiCCO (continuous cardiac output monitoring) , The two groups were observed during the operation of the body temperature is likely to reduce the four stages, namely, the beginning of general anesthesia, ECMO flow start stage, for lung transplantation stage, graft lung body temperature changes during the open phase. Results Comparing the two groups of patients in the beginning of general anesthesia, the beginning of ECMO, the stage of lung transplantation, the stage of lung transplantation, the body temperature on Pi CCO monitor, the differences were statistically significant (P <0.05) The experimental group patients were maintained at above 36 ℃ body temperature. Conclusion ECMO-assisted lung transplantation can effectively maintain the patient’s body temperature within the normal range in ECMO-assisted lung transplantation, which is instructive for the nursing of the operating room and ensures the safety of ECMO-assisted lung transplantation.
