目的: 探讨2 型糖尿病者胰岛素抵抗(IR) 与肾病变及血压的关系。方法: 测定72例2 型糖尿病伴或不伴肾病变者的胰岛素敏感指数 (葡萄糖利用常数, KI)、尿清蛋白排泄率 (UAER), 进行24 小时动态血压监测(ABPM)。结果: 胰岛素敏感指数K1 与UAER呈负相关, 糖尿病肾病变者胰岛素抵抗、血压升高(尤其是夜间血压升高) 及血压昼夜节律异常程度均显著高于不伴肾病变者。结论: 胰岛素抵抗与肾病变及血压升高关系密切, 三者相互影响并加重病情
Objective: To investigate the relationship between insulin resistance (IR) and nephropathy and blood pressure in type 2 diabetic patients. Methods: The insulin sensitivity index (KI) and urinary albumin excretion rate (UAER) were measured in 72 patients with type 2 diabetes mellitus or without renal disease and 24 hours ambulatory blood pressure monitoring (ABPM). Results: Insulin sensitivity index K1 was negatively correlated with UAER. Insulin resistance, elevated blood pressure (especially nocturnal elevation of blood pressure) and abnormal circadian rhythm of blood pressure in diabetic nephropathy patients were significantly higher than those without nephropathy. Conclusion: Insulin resistance is closely related to renal disease and high blood pressure, which affect each other and aggravate the disease
