目的:应用步态分析,观察Q4情景互动式康复训练系统对痉挛型偏瘫型脑瘫患儿步态时空参数和对称性参数的影响。方法选取痉挛型偏瘫型脑瘫患儿40例,按随机数字表法将其分为观察组及对照组,每组20例。两组均进行常规治疗,观察组在此基础上给予Q4情景互动式康复训练系统进行训练。分别于治疗前和治疗12w后使用三维步态分析仪器检测并获得两组患儿的步态参数。结果治疗12w后,两组患儿步频、步幅、步速、患侧摆动相和健侧摆动相均较治疗前明显均提高(<0.01),步宽、步态周期、双支撑相、患侧支撑相、健侧支撑相、步长偏差、健侧患侧支撑相比值和患侧健侧摆动相比值均较治疗前显著减小(<0.01)。组间比较显示,观察组患儿的步频、步幅、步速、步宽、步态周期、双支撑相、健侧支撑相、健侧摆动相、步长偏差、健侧患侧支撑相比值和患侧健侧摆动相比值改善均明显优于对照组(<0.05或0.01)。结论 Q4情景互动式康复训练系统联合康复训练能有效改善痉挛型偏瘫型脑瘫患儿步态时空参数和对称性参数,提高痉挛型偏瘫型脑瘫患儿的步行功能和步态的对称性。“,”Objective To observe the ef ect of Q4 scene interactive training system on temporal-spatial and symmetry parameters of gait of children with spastic hemiplegic cerebral palsy (CP) through gait analysis. Methods Forty children with spastic hemiplegic CP were randomly divided into observation group and control group with 20 cases in each group. Al children were treated with regular rehabilitation training. The observation group was received Q4 scene interactive training system in addition. Gait parameters were col ected and calculated by gait analysis system at the beginning and at the end of the 12th week of treatment respectively. Results Step frequency, step size, walking velocity, paretic swing phase and nonparetic leg swing phase significantly increased compared with pre-treatment in both groups at the end of 12th week of treatment ( <0.01);Stride width, gait cycle, double limb support phase, paretic stance phase, nonparetic stance phase, step dif erence, the ratio of nonparetic stance phase to paretic stance phase and the ratio of paretic swing phase to nonparetic leg swing phase significantly decreased compared with pre-treatment in both groups at the end of the 12th week of treatment ( <0.01). The parameters improved more significantly in observation group than those in control group at the end of the 12th week of treatment, including in step frequency, step size, walking velocity, stride width gait cycle, double limb support phase, nonparetic stance phase, nonparetic leg swing phase, step dif erence, the ratio of nonparetic stance phase to paretic stance phase and the ratio of paretic swing phase to nonparetic leg swing phase ( <0.05 or 0.01).Conclusion Q4 scene interactive training system in combination with rehabilitation functional training can improve the temporal-spatial and symmetry parameters of gait of children with spastic hemiplegic CP. So Q4 scene interactive training system can improve gait function and gait symmetry.
