目的研究老年胸部肿瘤放化疗对心电图的影响。方法选择该院就诊的150名胸部恶性肿瘤患者的临床资料,按照治疗方式不同分为CTX(环磷酰胺化疗)组、RT(放疗)组以及CTX+RT(环磷酰胺化疗+放疗)组,进行24 h动态心电图监测,观察最高心率、最低心率、平均心率,发生心律异常例数,期前收缩以及心动过速出现的次数。结果 CTX+RT组最低心率(69.38±5.84)次/分、最高心率(108.75±8.03)次/分、24 h平均心率(93.03±9.58)次/分;发生ST-T段改变8例、心律不齐11例、传导阻滞13例、QRS波压低5例,总异常率74%;发生房性期前收缩(110.38±9.02)次、室性期前收缩(89.03±9.83)次、房性心动过速(8.33±0.98)次、室性心动过速(5.67±0.69)次。结论联合使用放化疗治疗胸部肿瘤会导致明显的心脏损害,引起心率加快、ST-T段改变、传导阻滞、QRS波压低以及期前收缩、心动过速等心律失常的发生。
Objective To study the effect of chemoradiotherapy on electrocardiogram in elderly patients with thoracic tumors. Methods The clinical data of 150 patients with thoracic malignant tumor treated in this hospital were divided into CTX (cyclophosphamide) chemotherapy group, RT (radiotherapy) group and CTX + RT (cyclophosphamide chemotherapy + radiotherapy) group according to different treatment methods. The 24-hour Holter monitoring was performed to observe the maximum heart rate, the lowest heart rate, the average heart rate, the number of cases with abnormal heart rhythm, the premature rupture and the number of tachycardia occurred. Results The lowest heart rate (69.38 ± 5.84) / min, the highest heart rate (108.75 ± 8.03) / min and the mean heart rate at 24 h (93.03 ± 9.58) / CTX + RT group were significantly higher in CTX + RT group 11 cases of missing, conduction block in 13 cases, QRS wave pressure in 5 cases, the total abnormal rate of 74%; occurred atrial contraction (110.38 ± 9.02) times, ventricular contraction (89.03 ± 9.83) times, atrial Tachycardia (8.33 ± 0.98) times, ventricular tachycardia (5.67 ± 0.69) times. Conclusion Radiotherapy combined with chemoradiotherapy in the treatment of thoracic tumors can lead to obvious cardiac damage, which leads to heart rate, changes of ST-T segment, conduction block, low pressure of QRS wave and arrhythmia such as premature rupture and tachycardia.
