目的探讨半导体激光定位仪对CT引导穿刺射频热凝术治疗三叉神经痛的辅助作用。方法将67例原发性三叉神经痛患者随机分组:对照组(35例)在CT定位下经皮卵圆孔穿刺行三叉神经射频热凝术,研究组(32例)应用半导体激光定位仪辅助CT定位穿刺进行射频热凝术,比较两组患者的首次穿刺成功率、疼痛治疗效果及并发症等。结果对照组和研究组首次穿刺成功率分别为54%(19/35)和78%(25/32),首次穿刺成功率间差别有统计学意义(P<0.05)。对照组术后优良28例,好转4例,无效3例;研究组术后优良26例,好转3例,无效3例,两组治疗效果间差别无统计学意义(P>0.05)。两组患者均未出现严重的治疗相关并发症。结论应用半导体激光定位仪可提高三叉神经穿刺的成功率,操作方便、使用安全,具有较高的临床应用价值。
Objective To investigate the auxiliary effect of semiconductor laser locator on the treatment of trigeminal neuralgia with CT-guided radiofrequency thermocoagulation. Methods Thirty-seven patients with primary trigeminal neuralgia were randomly divided into control group (35 cases) underwent radiofrequency trigeminal percutaneous radiofrequency ablation under CT localization. The study group (32 cases) was assisted by a semiconductor laser locator CT positioning puncture radiofrequency thermocoagulation, two sets of patients compared the success rate of the first puncture, pain treatment and complications. Results The success rates of the first puncture in the control group and the study group were 54% (19/35) and 78% (25/32), respectively. The difference between the first puncture success rates was statistically significant (P <0.05). In the control group, 28 cases were excellent, 4 cases improved and 3 cases were ineffective. The study group was excellent in 26 cases, improved in 3 cases and ineffective in 3 cases. The difference between the two groups was not statistically significant (P> 0.05). No serious treatment-related complications occurred in both groups. Conclusion The application of semiconductor laser positioning device can improve the success rate of trigeminal nerve puncture, easy operation, safe use and high clinical value.
