目的:旨在使用Meta分析方法系统评价八聚体结合转录因子4(octamer-binding transcription factor 4,OCT4)表达与非小细胞肺癌(non-small cell lung cancer,NSCLC)临床病理特征的关系。方法:电子检索Pub Med、Embase、中国生物医学文献数据库、中国学术期刊全文数据库、维普中文科技核心全文数据库以及万方数据库,检索时间从建库至2016年3月,查找涉及OCT4表达与NSCLC临床病理特征相关的病例对照研究。参照Cochrane协作网推荐的方法进行筛选及提取数据并评价纳入研究的质量,运用Revman 5.2软件及Stata 12.0软件进行Meta分析。结果:共纳入10个研究,905例患者。OCT4阳性组471例,OCT4阴性组434例。Meta分析结果显示,OCT4阳性表达与肺腺癌(OR=1.75,95%CI=1.22~2.53,P=0.003)、低分化癌(OR=2.93,95%CI=1.25~6.84,P=0.010)、中分化癌(OR=1.81,95%CI=1.12~2.94,P=0.020)、T3/T4期肿瘤(OR=1.95,95%CI=1.10~3.45,P=0.020)、术后生存期短(鳞癌:MD=-4.49,95%CI=-7.73~-1.55,P=0.003;腺癌:MD=-8.52,95%CI=-9.82~-7.23,P=0.000)及总体生存率低下(HR=1.99,95%CI=1.52~2.60)关系较为密切,差异均具有统计学意义。结论:OCT4阳性表达与肺腺癌、NSCLC恶性特征以及预后不良密切相关,可以为肺癌靶向治疗提供可靠的科学依据。
OBJECTIVE: To systematically evaluate the relationship between the expression of octamer-binding transcription factor 4 (OCT4) and the clinicopathological features of non-small cell lung cancer (NSCLC) using meta-analysis. Methods: Electronic search Pub Med, Embase, China Biomedical Literature Database, Chinese Academic Journal Full-text Database, VIP Chinese Science Core Full-text Database and Wanfang Database, search time from Jiankui to March 2016, search for OCT4 expression related to clinical NSCLC Pathological features associated with case-control studies. Based on the methods recommended by the Cochrane Collaboration, screening and data extraction were performed and the quality of the included studies was evaluated. Meta analysis was performed using Revman 5.2 software and Stata 12.0 software. Results: A total of 10 studies and 905 patients were included. There were 471 OCT4-positive patients and 434 OCT4-negative patients. Meta-analysis showed that OCT4 positive expression was associated with lung adenocarcinoma (OR=1.75, 95%CI=1.22~2.53, P=0.003), poorly differentiated cancer (OR=2.93, 95%CI=1.25~6.84, P=0.010). Mean, moderately differentiated carcinoma (OR=1.81, 95%CI=1.12~2.94, P=0.020), T3/T4 stage tumors (OR=1.95, 95%CI=1.10~3.45, P=0.020), short postoperative survival period (squamous cell carcinoma: MD=-4.49, 95% CI=-7.73 to -1.55, P=0.003; adenocarcinoma: MD=-8.52, 95% CI=-9.82 to -7.23, P=0.000) and low overall survival rate (HR=1.99, 95%CI=1.52~2.60) The relationship was relatively close and the differences were statistically significant. Conclusion: The positive expression of OCT4 is closely related to the malignant characteristics of lung adenocarcinoma, NSCLC and poor prognosis. It can provide a reliable scientific basis for the targeted treatment of lung cancer.
