目的总结糖尿病足的临床特点及治疗体会,探讨以内分泌科为主的糖尿病足治疗模式。方法回顾分析南通大学附属医院内分泌科2012年01月至2012年12月,收治的WagnerⅢ～Ⅳ级糖尿病足12例。总结内科综合治疗基础上,根据病情分阶段有重点地进行床旁清创治疗的效果。结果 12例WagnerⅢ～Ⅳ级糖尿病足患者,临床特点为年龄高、病程长、并发症和合并症多,在代谢紊乱的基础上合并营养不良。治愈9例,死亡1例,截趾1例,截肢1例。结论无需紧急外科手术治疗的糖尿病足患者,内科综合治疗联合床旁清创可有效促进创面愈合,降低截肢率。
Objective To summarize the clinical features and treatment experience of diabetic foot and to explore the treatment mode of diabetic foot with endocrinology as the mainstay. Methods Retrospective analysis of Nantong University Hospital Endocrinology January 2012 to December 2012, admitted to Wagner Ⅲ ~ Ⅳ grade diabetic foot in 12 cases. Based on the comprehensive medical treatment, based on the disease stage by stage focused on the effect of debridement treatment. Results The clinical features of 12 Wagner Ⅲ ~ Ⅳ diabetic foot patients were high age, long course of disease, more complication and complications, malnutrition on the basis of metabolic disorders. Nine cases were cured, one died, one was cut off, and one was amputated. Conclusion Without emergency surgical treatment of diabetic foot patients, combined medical treatment with bedside debridement can effectively promote wound healing, reduce amputation rate.
