目的探讨万古霉素致急性肾功能损害的发生机制及高危因素。方法对我院近期发生的1例万古霉素致急性肾功能损害的不良反应进行因果关系分析,并查阅相关文献进行分析讨论。结果该患者治疗过程中存在万古霉素致肾功能损害危险因素。肾功能损害为万古霉素已知不良反应,但其发生于用药后第4天,较其他文献报道的第6天早。结论肾功能损害是万古霉素的主要不良反应之一,临床应用万古霉素时应开展全方位的药学监护。 Objective To investigate the pathogenesis and risk factors of acute renal damage caused by vancomycin. Methods A causal analysis was conducted on the adverse reactions of vancomycin-induced acute renal impairment in our hospital recently. The related literature was reviewed and analyzed. Results There was a risk factor of renal damage due to vancomycin during the course of treatment. Renal dysfunction was a known adverse event of vancomycin, but it occurred on day 4 after dosing as early as day 6 reported in other literature. Conclusion Renal dysfunction is one of the major adverse reactions of vancomycin. Clinical application of vancomycin should be carried out in all aspects of pharmacy care.
