目的:探讨在无明显机械压迫的情况下,髓核自身在腰腿痛发病中的作用。方法:将大鼠自身的尾椎髓核取出置于硬膜外腔,造成非压迫性的大鼠硬膜外自体髓核移植模型,并对其热刺激回缩潜伏期及机械刺激回缩阈值进行测定。结果:即使在无明显机械压迫的情况下,大鼠硬膜外移植自体髓核后可产生明显的痛觉过敏行为。结论:髓核自身是引起腰腿痛的重要原因,推测髓核中的液体漏出刺激神经根和硬膜可能是临床上产生腰腿痛的机制之一。
Objective: To investigate the role of the nucleus pulposus itself in the pathogenesis of low back pain in the absence of obvious mechanical compression. Methods: The rat caudate nucleus pulposus was removed and placed in the epidural space, resulting in a non-compressive rat model of epidural autologous nucleus pulposus transplantation. Results: Significant hyperalgesia occurred after epidural transplantation of autologous nucleus pulposus even in the absence of obvious mechanical compression. Conclusion: The nucleus pulposus itself is an important cause of low back pain. It is speculated that leakage of fluid from the nucleus pulposus may stimulate the nerve root and dura mater and may be one of the mechanisms of clinically low back pain.
