甲状腺肿瘤的发生,除了伴随着T3、T4、TSH等内分泌激素的一系列变化外,还与多种细胞因子的作用有着密切关系。本文介绍了表皮生长因子(EGF)、血小板源性生长因子(PDGF)、转化生长因子(TGF-α、TGF-β1)胰岛素样生长因子-1(IGF-1)及其各自受体与甲状腺肿瘤细胞生长的关系,以及各细胞因子之间的相互作用关系。 细胞因子与甲状腺肿瘤的关系 EGF及其受体 EGF与多种组织细胞的生长刺激作用有关,表皮生长因子受体(EGF-R)则是癌基因G-erbB-1的表达产物,它与相应配体EGF结合引起细胞过度分裂与增殖。研究发现,EGF的促甲状腺肿瘤细胞生长作用与EGF及其受体在细胞内的定位有关。在正常甲状腺组织中,EGF/EGF-R存在于甲状腺滤泡上皮细胞胞质中。在Graves’病患者中,EGF/EGF-R胞质染色加深,而核染色阳性则是甲状腺滤泡状癌及腺瘤的特征。在乳头状癌及腺瘤细胞中,EGF/EGF-R核染色为阴性。此外,免疫组化染色显示,在分化性甲状腺癌中,EGF-R阳性表达率达61.2%,其中乳头状癌62.5%,滤泡状癌61%,均高于甲状腺良性癌变(P<0.05)。尤其是伴有淋巴转移的甲状腺癌,其表达率高达83.9%,而无淋巴转移者为48.6%,(P<0.05)。提示EGF/EGF-R
