测定57例不同胎龄新生儿的肺功能和脐血中皮质醇及脐血总蛋白水平。结果:早产儿、足月儿和过期产儿的脐血总蛋白水平呈阶梯状上升。早产儿脐血皮质醇显著低于足月儿,而足月儿与过期产儿差异不显著。经相关分析脐血皮质醇与总蛋白呈正相关,胎龄<37周者相关极显著(r=0.924,P<0.01),>37周者相关无显著意义(r=-0.17,P>0.05)。过期产儿的各种肺功能指标均与足月儿接近,早产儿CVC和Vmax较足月儿低。CVC和Vmax与脐血总蛋白和皮质醇均有显著相关。结果表明:脐血总蛋白和皮质醇水平可作为反映胎儿成熟度和肺成熟度的指标;肺成熟度随胎儿的成熟度逐渐上升。
The lung function and total protein levels of cortisol and cord blood in 57 neonates with different gestational age were determined. Results: The levels of total umbilical cord blood protein in preterm infants, full-term infants and expired infants increased stepwise. Cord blood cortisol in preterm infants was significantly lower than that in term infants, whereas there was no significant difference between term infants and expired infants. Correlation analysis showed that there was a positive correlation between cortisol and total protein in umbilical cord blood (P <0.01). There was no significant correlation between cord blood cortisol and total protein (r = -0.17, P> 0.05) . Outdated children of various lung function indicators are close to full-term children, premature children CVC and Vmax lower than full-term children. CVC and Vmax with cord blood total protein and cortisol were significantly related. The results showed that the level of total protein and cortisol in umbilical cord blood could be used as an indicator of fetal maturity and lung maturity. The degree of lung maturity increased with the maturity of the fetus.
