目的 进一步探讨超声引导下微波介入治疗肝癌的可行性和疗效。方法 对 43例中晚期肝癌患者进行超声引导下微波治疗。采用分次多点多部位由深到浅低功率长时间分段凝固 ,输出功率 30~ 6 0W ,作用时间 2 0 0~ 1 80 0s。结果 所有病例治疗后肿块缩小 ,其中 3例完全消失 ,肿块中央回声增强 ,周边呈低回声 ,境界清晰 ,肿瘤内血流消失或减少 ;1 2例CT检查示肿块缩小 ,无强化 ;2 3例超声引导下活检提示大片坏死。 43例均健在。随访最长已 1 0个月 ,治疗后全身情况好转 ,症状减轻或消失 ,患者饮食明显增加 ( 33例 ) ,AFP和肝功能改善 ,生活质量提高 ,无严重并发症发生。结论 超声引导微波治疗中晚期肝癌有较好的疗效 ,适于各期肝癌的治疗。
Objective To further explore the feasibility and efficacy of ultrasound-guided microwave interventional therapy for liver cancer. Methods Ultrasound-guided microwave therapy was performed on 43 patients with advanced liver cancer. It adopts fractional multi-points and multi-sites for deep-to-low-power segmented solidification for a long time, with an output power of 30 to 60W and a working time of 200 to 180 hours. Results After treatment, the tumors were reduced in all cases. Three of them disappeared completely. The central echo of the tumor was enhanced. The periphery was hypoechoic and the realm was clear. The blood flow in the tumor disappeared or decreased. The CT examination showed that the tumor was reduced and no enhancement was observed in 23 cases. Ultrasound-guided biopsy revealed massive necrosis. 43 cases were alive. The longest follow-up period was 10 months. After the treatment, the general condition improved, the symptoms decreased or disappeared, the patient’s diet increased significantly (33 cases), AFP and liver function improved, quality of life improved, and no serious complications occurred. Conclusion Ultrasound-guided microwave therapy is effective for the treatment of advanced liver cancer. It is suitable for the treatment of liver cancer in each stage.
