目的:研究糖尿病病人在合并肾病时血脂,载脂蛋白的变化情况,指导临床工作。方法:选择健康人60例及我院住院病人72例,根据24h尿白蛋白定量确定糖尿病无肾病组39例,早期肾病组33例,全部病人观测甘油三酯、胆固醇、高密度脂蛋白、低密度脂蛋白、载脂蛋白A、载脂蛋白B,并将糖尿病早期肾病组各项指标与正常人及糖尿病无肾病组各项指标分析比较。结果:糖尿病早期肾病病人甘油三酯,低密度脂蛋白明显高于正常对照组及糖尿病无肾病组(P<0.05)。早期肾病组高密度脂蛋白明显低于正常对照组及糖尿病无肾病组(P<0.05)。早期肾病组胆固醇明显高于正常对照组(P<0.05),略高于糖尿病无肾病组,二者相比有统计学意义(P<0.05)。早期肾病组apoA与正常对照组比下降显著(P<0.05),与无肾病组比轻度下降,apoB与正常对照组相比显著升高(P<0.05),与无肾病组相比轻度升高。结论:糖尿病早期肾病病人血脂水平、载脂蛋白与正常对照组及糖尿病无肾病组差异显著,对于糖尿病合并肾病病人更应严格控制血脂水平。
Objective: To study the changes of blood lipids and apolipoproteins in diabetic patients with nephropathy and to guide the clinical work. Methods: Sixty healthy subjects and 72 inpatients were selected. Thirty-nine patients with no nephropathy and 33 patients with early nephropathy were enrolled in this study. The levels of triglyceride, cholesterol, high density lipoprotein, Density lipoprotein, apolipoprotein A and apolipoprotein B. The indexes of early diabetic nephropathy group were compared with those of normal people and non-diabetic nephropathy group. Results: The levels of triglyceride and low density lipoprotein in patients with early diabetic nephropathy were significantly higher than those in normal control group and diabetic nephropathy group (P <0.05). Early nephropathy HDL was significantly lower than the normal control group and diabetic nephropathy group (P <0.05). The cholesterol in early nephropathy group was significantly higher than that in normal control group (P <0.05), slightly higher than that in non-diabetic nephropathy group (P <0.05). The ratio of apoA to normal control in early nephropathy group was significantly lower than that in normal control group (P <0.05), slightly decreased compared with that in non-nephropathy group, apoB was significantly higher than that in normal control group (P <0.05) Rise. Conclusion: There is significant difference between the levels of serum lipids, apolipoproteins in normal diabetic patients and non-diabetic nephropathy patients with diabetic nephropathy. Serum lipids should be controlled strictly in patients with diabetic nephropathy.
