脑转移是肺癌的严重并发症。回顾性研究了组织学确诊的50例非小细胞肺癌(NSCLC)伴脑转移患者,并评估其预后。患者及方法 50例患者中39例为腺癌,7例为鳞癌,4例为大细胞肺癌。采用一般状态(GPS)及神经功能分级(NFC)来判断患者状态及治疗情况。治疗包括支持疗法、全脑放射治疗(WBRT)、化疗(CT)、脑肿瘤切除术(BTR),脑室—腹膜(VP)分流术。结果 14例单发脑转移患者平均存活期为7个月,36例多发性脑转移患者为3个月。50例中9例应用BTR治疗后平均存活期为9个月,10例经CT治疗后为8个月,22例接受WBRT治疗后为7个月,而17例采用支持疗法的患者则病情迅速恶化,其中75%
Brain metastases are a serious complication of lung cancer. A retrospective study of 50 histologically confirmed patients with non-small cell lung cancer (NSCLC) with brain metastases and their prognosis were evaluated. Patients and Methods Of the 50 patients, 39 were adenocarcinoma, 7 were squamous cell carcinoma, and 4 were large cell lung cancer. General status (GPS) and neurological function classification (NFC) were used to judge the patient status and treatment status. Treatments included supportive therapy, whole-brain radiotherapy (WBRT), chemotherapy (CT), brain tumor resection (BTR), and ventricle-peritoneal (VP) shunt. Results The average survival time of 14 patients with single brain metastases was 7 months, and 36 patients with multiple brain metastases were 3 months. Nine out of 50 patients treated with BTR had an average survival of 9 months, 10 patients were treated with CT for 8 months, 22 patients received WBRT after treatment for 7 months, and 17 patients with supportive therapy had rapid disease.
