目的探讨手术不同时机应用充气加温毯对结肠癌患者体温及预后的影响。方法选取2010年6月至2015年9月间内蒙古医科大学第一临床附属医院收治的488例行手术治疗的结肠癌患者,采用随机数字表法分为观察组与对照组,每组244例。观察组患者进入手术间时立即采用充气加温毯并保温至术毕,对照组患者在麻醉诱导时给予充气加温毯保温至术毕。观察两组患者不同时间点的体温变化、相关手术指标及术后并发症发生情况。结果观察组患者T2、T5和T6时间点体温均高于T1点,差异均有统计学意义(均P<0.05);对照组患者T2~T6时间点体温均低于T1点,除T6点外,差异均有统计学意义(均P<0.05)。观察组患者T2~T6时间点体温均高于对照组,差异均有统计学意义(均P<0.05)。观察组患者麻醉苏醒时间、寒战和苏醒期躁动发生率均明显低于对照组,差异均有统计学意义(均P<0.05)。两组术中出血量、术后感染、心率、平均动脉压及恶心呕吐发生率比较,差异均无统计学意义(P>0.05)。结论术前使用充气式保温毯可减少围手术期低体温及相关并发症的发生,有效改善患者预后,值得临床推广。
Objective To investigate the effect of inflatable warm blanket on the body temperature and prognosis of patients with colon cancer at different times of operation. Methods A total of 488 patients with colorectal cancer who underwent surgery from June 2010 to September 2015 in the First Clinical Affiliated Hospital of Inner Mongolia Medical University were divided into observation group and control group with random number table method, with 244 cases in each group. Patients in the observation group were inflated with warm blankets and kept warm until the operation was completed. Patients in the control group were given warm air-filled blankets during the induction of anesthesia. The changes of body temperature at different time points, the related operation indexes and the incidence of postoperative complications were observed. Results In the observation group, the body temperature at T2, T5 and T6 were all higher than those at T1, the differences were statistically significant (all P <0.05); in the control group, the body temperature at T2 ~ T6 was lower than T1, except T6 , The differences were statistically significant (all P <0.05). The body temperature in observation group was higher than that in control group at T2 ~ T6 (all P <0.05). The observation group anesthesia recovery time, chills and wakefulness agitation were significantly lower than the control group, the difference was statistically significant (P <0.05). There were no significant differences in the incidence of blood loss, postoperative infection, heart rate, mean arterial pressure and nausea and vomiting between the two groups (P> 0.05). Conclusion Preoperative use of inflatable insulation blanket can reduce perioperative hypothermia and related complications, effectively improve the prognosis of patients, it is worth clinical promotion.
