目的:探讨甲状腺激素分泌异常和急性缺血性卒中相关危险因素、卒中严重性、TOAST及OCSP分型之间的关系。方法收集220例急性缺血性卒中患者,将其分为甲状腺激素分泌正常组(n=143)、甲状腺激素分泌增高组(n=49)及甲状腺激素分泌减低组(n=28)。甲状腺素(T3、T4、FT3、FT4及TSH)水平检测采用放免法。结果(1)甲状腺激素分泌减低组及增高组平均年龄、纤维蛋白原水平及C反应蛋白水平高于正常组(P<0.05);(2)甲状腺激素分泌减低组及增高组NIHSS评分明显高于正常组(P=0.003);(3)甲状腺激素分泌减低组及增高组完全前循环梗死(TACI)比例明显高于正常组,部分前循环梗死(PACI)低于正常组,各组比较差异有统计学意义(P=0.041);(4)甲状腺激素分泌减低组及增高组大动脉粥样硬化型(LAA)及心源性栓塞型(CE)比例高于正常组,小动脉病变型(SAO)比例低于正常组,但差异无统计学意义(P=0.273)。结论急性缺血性卒中患者中甲状腺激素分泌异常(增高或者减低)多见于高龄、高纤维蛋白原血症及高C反应蛋白患者。甲状腺激素分泌异常患者伴有高的卒中严重性及高的TACI型比例。“,”Objective To investigate the relationship between simply abnormal thyroid hormone level and related risk factors in acute ischemic stroke, stroke severity, TOAST and OCSP classification.Methods 220 cases with acute ischemic stroke were included in our study, and patients with acute ischemic stroke can be divided into normal thyroid hormone group (143 cases), increased thyroid hormone group (49 cases) and reduced thyroid hormone group (28 cases). The hormonal levels of T3, T4, FT3, FT4 and TSH were detected by radioimmunoassay.Results (1)The average age, concentration of ifbrinogen and C-reactive protein levels of the low and high groups were higher than that of the normal group (P<0.05); (2)The NIHSS score of the low and high group was signiifcantly higher than that of the normal group (P=0.003); (3)The proportion of total anterior circulation infarct (TACI) in the low group and high group was signiifcantly higher than that of the normal group,the proportion of partial anterior circulation infarct (PACI) in the low and high group was higher than that of the normal group,the differences between the three groups were statistically signiifcant (P=0.041); (4)The ratio of large artery atherosclerosis (LAA) and cardio embolism (CE) in the low and high group were higher than the normal group, small artery occlusion (SAO) ratio was lower than the normal group, but the differences were not statistically significant (P=0.273).Conclusion Acute ischemic stroke patients with abnormal thyroid hormone levels (higher or lower) usually occurs in patients with older age, high blood plasma ifbrinogen and high C-reactive protein. The patients with abnormal thyroid hormone are associated with high stroke severity and high proportion of TACI type.
