【目的】分析老年腹股沟疝患者的肌肉状况。【方法】选取2010年1月至2015年1月本院收治的90例腹股沟疝患者为观察组,选取同期90例65~85岁男性健康体检者为对照组,进行人体测量、最大肌力测试,采用生物电阻抗和双能 X 线扫描的方法测定人体组分、骨骼肌肉指数,CT 扫描腹部和大腿,并计算相应肌肉面积及密度。【结果】老年腹股沟疝组瘦组织质量和腰臀脂肪比率分别为(46.29±7.06)kg、(0.98±0.51)%,与对照组有明显差异(P <0.05)。腹股沟疝组患者仰卧推举最大肌力显著低于对照组,差异有统计学意义(P <0.05)。腹股沟疝组患者骨骼肌肉指数(7.12±0.84)kg/m2,显著低于对照组(P <0.05);肌肉减少症发病率为31.11%,显著高于对照组18.89%(P <0.05)。CT 扫描发现老年组腹壁肌肉和大腿肌肉均少于对照组,但竖脊肌结果两组相近,腹部肌肉密度明显低于对照组(P <0.05)。【结论】老年腹股沟疝患者肌肉的状况比同龄健康人群差,肌肉减少症发生率增高;尤其是腹部肌肉面积及密度显著降低。“,”Objective]To analyze the muscle situation in elderly patients with inguinal hernia.[Methods]Ninety elderly patients with inguinal hernia were selected for the study in our hospital from January 2010 to Janu-ary 201 5,and 90 healthy male cases aged 65 to 85 year-old were selected as healthy control group in the same peri-od.Human body measurement and maximum muscle strength tests were conducted.Body composition and skeletal muscle index were tested by bioelectrical impedance and dual energy X-ray scanning.Abdomen and thigh muscles were scanned by CT and the relevant muscle area and density was calculated .[Results]Lean tissue mass and fat-hip ratio in the guinal hernia group were (46.29 ± 7.06)kg、(0.98 ±0.5 1 )% respectively ,which had significant difference with those of the control group (P <0.05).The maximum muscle strength of the patients with the in-guinal hernia group was significantly lower than that of the control group (P <0.05).Skeletal muscle index (7.12 ±0.84)kg/ m2 in patients with inguinal hernia was significantly lower than that in the control group (P <0.05). The incidence of muscle reduction was 18.89%,which was significantly higher than that of the control group (P<0.05).CT scanning showed that the abdominal muscle and thigh muscle in the elderly group were less than those in the control group,but the vertical spinal muscular results were similar in the two groups,and the abdominal muscle density was significantly lower than that in the control group (P <0.05 ).[Conclusions]Muscle status of elderly patients with inguinal hernia is worse than that of their health peers.The incidence of muscle reduction in elderly patients is increased,and the abdominal muscle size and density are reduced.
