目的探讨肝细胞癌(HCC)微血管和成熟血管的特征及其关系。方法选取行手术切除并经病理证实的HCC 31例,检测VEGF、Flk-1、PCNA的表达情况,同时对微血管和成熟血管的数目、平均面积、总面积、周长、直径、异型指数、血管间距、表达部位,以及动脉数、静脉数、动脉比、静脉比、血管成熟指数和平均灌注分数等进行统计。HE染色计算细胞外间隙及细胞外血管外间隙。分析微血管和成熟血管特征差异及其相互关系。结果HCC微血管的数目、总面积、异型指数均大于成熟血管,而平均面积、直径和血管间距则相反(P<0.05)。在VEGF阴、阳性表达组之间,微血管的数目、周长及细胞外间隙均存在统计学差异(P<0.05),但各成熟血管参数未见统计学差异(P>0.05)。在Flk-1阳性和阴性表达组,微血管的数目、异型指数、周长以及动脉数也存在统计学差异(P<0.05)。另外,PCNA与微血管异型指数呈正相关(P=0.045,r=0.563),与微血管间距负相关(P=0.022,r=-0.627)。其余测量参数之间差异未见统计学意义(P>0.05)。结论HCC微血管和成熟血管特征存在较大差别,VEGF及Flk-1主要参与微血管生成的调控,对成熟血管作用不大。
Objective To investigate the characteristics and relationship of microvascular and mature blood vessels in hepatocellular carcinoma (HCC). Methods Thirty-one cases of HCC confirmed by pathology were selected to detect the expression of VEGF, Flk-1 and PCNA. The number of microvascular and mature blood vessels, the average area, the total area, the circumference, the diameter, the atypia index, Distance, expression site, as well as the number of arteries, veins, artery ratio, venous ratio, vascular maturation index and average perfusion score and other statistics. HE staining to calculate the extracellular space and extracellular extravascular space. Analysis of microvascular and mature blood vessel characteristics differences and their relationship. Results The number of HCC microvessels, the total area and the atypia index were all larger than that of mature blood vessels, while the mean area, diameter and vessel spacing were opposite (P <0.05). The number of microvessels, perimeter and extracellular space were significantly different between the negative and positive expression of VEGF (P <0.05), but there was no significant difference between the parameters of mature blood vessels (P> 0.05). In the Flk-1 positive and negative expression groups, the number of microvessels, atypia index, perimeter, and number of arteries were also significantly different (P <0.05). In addition, there was a positive correlation between PCNA and microvascular atherosclerosis index (P = 0.045, r = 0.563) and a negative correlation with microvascular distance (P = 0.022, r = -0.627). The difference between the other measurement parameters was not statistically significant (P> 0.05). Conclusions The characteristics of microvascular and mature blood vessels in HCC are quite different. VEGF and Flk-1 are mainly involved in the regulation of microvascular angiogenesis, and have little effect on mature blood vessels.
