目的:观察参附注射液对脓毒症大鼠心脏血流动力学的影响。方法:采用肓肠结扎穿孔法制备腹腔感染脓毒症动物模型。80只SD大鼠随机分为假手术组、脓毒症组、参附注射液低、高剂量组,每组20只。各实验组术后给予相应处理措施,观察术后6、18h各组的平均动脉压(MAP)、心率(HR)、心脏收缩功能指标(LVDP和+dP/dt)、心脏舒张功能指标(LVEDP和-dP/dt)。结果:与假手术组比较,脓毒症组术后6h HR显著升高(P<0.05),术后18h HR显著降低(P<0.01);术后6h和18h MAP、LVDP、+dP/dt、LVEDP、-dP/dt均显著下降(P均<0.01)。与脓毒症组比较,参附注射液低剂量组术后6h MAP、HR差异无统计学意义(P均>0.05),术后18h MAP和HR显著升高(P<0.05);参附注射液高剂量组术后6h HR显著降低(P<0.05),术后18h HR和术后6、18h MAP均显著升高(P均<0.01),参附注射液低、高剂量组术后6、18h的LVDP、+dP/dt、LVEDP、-dP/dt均显著升高(P<0.05或<0.01)。结论:参附注射液可升高脓毒症大鼠的平均动脉压,改善其心脏收缩和舒张功能;高剂量参附注射液作用更明显,并能改善心率。参附注射液能改善脓毒症大鼠心脏血流动力学的作用强度与剂量有关。
Objective: To observe the effect of Shenfu injection on cardiac hemodynamics in septic rats. Methods: The animal model of sepsis induced by peritoneal cavity was prepared by colonic puncturation and perforation method. Eighty SD rats were randomly divided into sham operation group, sepsis group and Shenfu injection low and high dose group, 20 rats in each group. The rats in each experimental group were given corresponding treatment after operation, mean arterial pressure (MAP), heart rate (HR), indexes of cardiac systolic function (LVDP and + dP / dt), indexes of diastolic function And -dP / dt). Results: Compared with the sham operation group, the HR of sepsis group increased significantly (P <0.05) at 6h after operation and the HR decreased significantly at 18h after operation (P <0.01); MAP, LVDP, + dP / dt , LVEDP and -dP / dt decreased significantly (all P <0.01). Compared with the sepsis group, there was no significant difference in MAP and HR at 6h after operation (all P> 0.05), MAP and HR at 18h after operation in Shenfu injection low dose group (P <0.05) (P <0.05), HR of 18 h after operation and MAP at 6 and 18 h after operation were significantly higher (P all <0.01), and those of low dose and high dose of Shenfu injection 6 h after operation , LVDP, + dP / dt, LVEDP, -dP / dt at 18h were significantly increased (P <0.05 or <0.01). Conclusion: Shenfu injection can increase the mean arterial pressure in septic rats, and improve the systolic and diastolic function of the rats. The effect of Shenfu Injection is more obvious and the heart rate can be improved. Shenfu injection can improve cardiac hemodynamics in septic rats intensity and dose-related.
