目的研究胱抑素C(CysC)含量在烧伤患者早期肾功能损害的预测价值。方法选取既往无肾病史烧伤患者97例为研究对象,根据烧伤面积再分为中轻度烧伤组、重度烧伤及时复苏组、重度烧伤延迟复苏组和40例体检健康人群作为对照组。分析入选对象血清中CysC、尿素氮(BUN)、肌酐(Cr)的浓度。结果各烧伤组检测的血清BUN和&浓度,只有在重度烧伤延迟复苏组中才显著高于其他3组差异有统计学意义(P<0.05)。而测定的血清CysC浓度从重度烧伤及时复苏组就开始明显高于对照组和中轻度烧伤组差异有统计学意义(P<0.05)且其在重度烧伤延迟复苏组与及时复苏组的比较中也有显著性差异有统计学意义(P<0.05)。结论大面积烧伤患者血清CysC浓度与烧伤程度、烧伤休克复苏时间和肾脏的损伤程度呈正相关血清CysC作为早期肾损伤的检测指标最为敏感。在诊断大面积重度烧伤引起的早期肾损害有重要的临床意义。
Objective To study the predictive value of cystatin C (CysC) content in the early stage of renal impairment in burn patients. Methods Ninety-seven burn patients with no history of nephropathy were selected as study objects. According to the burn area, they were further divided into moderate and severe burn group, timely burn group, severe delayed burn group and 40 healthy people as control group. The serum concentrations of CysC, BUN and Cr in the selected subjects were analyzed. Results The BUN and serum levels of BUN in each burn group were significantly higher than those in the other three groups (P <0.05) only in delayed burn group with severe burn. However, the serum CysC concentration was significantly higher than that of the mild burn group and the moderate burn group (P <0.05), and the difference was statistically significant between the severe burn delayed recovery group and the timely recovery group There are also significant differences were statistically significant (P <0.05). Conclusions Serum CysC concentration is positively correlated with the degree of burn, recovery time of burn shock and degree of renal injury in patients with massive burn. Serum CysC is the most sensitive indicator of early renal injury. In the diagnosis of severe acute burn injury caused by the early kidney has important clinical significance.
