目的 CAD/CAM氧化锆全瓷冠修复体对牙周组织健康状况的影响。方法选择南阳医学高等专科学校第一附属医院院择期行全冠修复的50例患者(78颗基牙)为研究对象,治疗组25例(39颗基牙)实施CAD/CAM氧化锆全瓷冠修复。对照组25例(39颗基牙)实施镍铬合金烤瓷冠修复。记录两组修复前、修复后3个月、修复后12个月的牙龈出血指数(SBI)、附着丧失(AL)、牙周袋深度(PD),并同期采集龈沟液,测量冠修复前后GCF含量及龈沟液IL-6、TNF-α水平。结果修复12个月后观察组修复体的完整性优于对照组(P<0.05)。两组患者修复体边缘与基牙密合性好,组间差异无统计学意义(P>0.05)。观察组修复体与邻牙颜色匹配度好,与对照组比较,差异有统计学意义(P<0.05)。观察组修复体牙龈健康,探诊无牙龈出血及萎缩,优于对照组,差异有统计学意义(P<0.05)。观察组修复前、后GCF含量、TNF-α、IL-6、SBI、AL及PD比较差异均无统计学意义(P>0.05)。对照组修复3个月、修复12个月后的GCF含量、TNF-α、IL-6、SBI、AL及PD均明显高于修复前(P>0.05)。而且修复3个月、修复12个月,对照组GCF含量、TNF-α、IL-6、SBI、AL及PD均高于观察组(P<0.05)。结论 CAD/CAM氧化锆全瓷冠修复增强冠修复的美学效果,其稳定性、耐腐蚀性、抗核磁干扰性、生物相容性均佳,有效降低修复体对牙龈的刺激,更有益于牙周组织的健康。
Objective To investigate the effects of CAD / CAM zirconia all ceramic crowns on the health status of periodontal tissues. Methods Fifty patients (78 abutments) undergoing elective crown restoration at the First Affiliated Hospital of Nanyang Medical College were enrolled in this study. Twenty-five patients (39 abutments) in the treatment group underwent CAD / CAM zirconia all-ceramic crowns repair. Control group 25 cases (39 abutments) implementation of nickel-chromium alloy porcelain crown repair. The SBI, AL and PD of the two groups were recorded before repairing, 3 months after repairing, 12 months after repairing, and the gingival crevicular fluid was collected at the same time, GCF content and gingival crevicular fluid IL-6, TNF-α levels. Results After 12 months of repair, the integrity of the prosthesis in the observation group was better than that in the control group (P <0.05). There was no significant difference between the two groups (P> 0.05). The color matching degree between the observation group and the adjacent teeth was good, the difference was statistically significant (P <0.05). The gingival health of the observation group prosthesis, exploration of gingival bleeding and atrophy were better than the control group, the difference was statistically significant (P <0.05). There was no significant difference in GCF content, TNF-α, IL-6, SBI, AL and PD before and after the repair in the observation group (P> 0.05). The GCF content in the control group after 3 months of repair and 12 months after repair was significantly higher than that before repair (P> 0.05). The levels of TNF-α, IL-6, SBI, AL, After 3 months repair and 12 months repair, GCF content, TNF-α, IL-6, SBI, AL and PD in the control group were higher than those in the observation group (P <0.05). CONCLUSION: The aesthetical effect of CAD / CAM zirconia all-ceramic crown restoration and crown restoration is good, its stability, corrosion resistance, anti-nuclear magnetic interference and biocompatibility are all good, which can effectively reduce the stimulus of the prosthesis to the gums, Weekly health organization.
