目的 介绍尿道下裂的治疗方法。方法 对于阴茎下弯矫正后尿道缺损短的尿道下裂病例 ,采用横裁带蒂岛状包皮瓣尿道成形术 (即Dutckett术式 )修复 ,对于尿道缺损长的重度尿道下裂者 ,采用Duckett术联合尿道口周围皮肤作Duplay尿道成形后修复 (即Duckett +Duplay术式 ) ,本组采用Duckett术式 2 7例 ,Duckett+Duplay术式 3例。结果 30例中 ,一次手术成功 2 5例 (83.3% ) ,尿瘘 4例 ,尿道狭窄合并尿瘘 1例 ,全部病例得到随访 ,时间 6～ 2 0个月 ,已治愈的患儿阴茎外观及排尿均良好。结论 Duckett术式是治疗尿道下裂的一种基本的较好的方法 ,尤其适用于伴有阴茎下弯的阴茎体型尿道下裂 ,可作为首选 ,对于重度尿道下裂 ,可选用Duckett +Duplay术式 ,尿瘘是其主要并发症。
Objective To introduce hypospadias treatment. Methods For the case of hypospadias with short urethral defect after correction of penile curvature, a modified pedicle island flap urethroplasty (Dutckett procedure) was used. For severe hypospadias with urethral defect, Duckett Duch urethral urethroplasty combined surgery around the skin for urethroplasty (ie, Duckett + Duplay operation), the group used Duckett operation 27 cases, Duckett + Duplay operation in 3 cases. Results Among 30 cases, 25 cases (83.3%) were successful in one operation, 4 cases of urinary fistula, 1 case of urethral stricture combined with urinary fistula, all cases were followed up for 6-20 months. The cured penis appearance and urination good. Conclusion Duckett operation is a basic and preferable method for the treatment of hypospadias. It is especially suitable for the treatment of hypospadias with hypospadias. It is the first choice. For severe hypospadias, Duckett + Duplay Urinary fistula is its main complication.
