目的 :通过研究云南佤族人群Rh、Duffy血型系统抗原表现型的分布、抗原频率与不规则抗体产生几率的关系,从而为云南少数民族人群血型分布的多态性提供科学依据。同时为该民族群体稀有血型库的建立、组织配型和免疫性输血反应的预防提供科学数据。方法:采集云南省耿马县3代内无血缘关系的佤族人群(男女各150例)血样5 m L(EDTA抗凝),采用微柱凝胶法检测Rh、Duffy及抗体筛选,Duffy血型系统基因分型检测采用序列特异引物引导的PCR反应(PCR-SSP),所得结果与当地汉族人群为对照,作对比分析。结果:300例佤族人中Rh D阳性率为99.0%,Rh D阴性率为1.0%;Duffy基因分型检测结果:Fya基因频率0.994 5,Fyb基因频率0.003 5,Fya-b+检出1例;佤族女性有5例抗体筛选检测为阳性,均为同种抗体(E抗体),不规则抗体检出率占女性人群的3.33%。结论:云南佤族人群的Rh、Duffy血型系统抗原分布状况及抗原分布特征与当地汉族人群基本相符,但也有本民族的特点,佤族人群(尤其是妇女)的不规则抗体检出率比当地汉族人群高。
OBJECTIVE: To study the distribution of antigenic phenotypes of Rh and Duffy blood group and the relationship between antigen frequency and the incidence of irregular antibodies in ethnic Yunnan Va population, and to provide a scientific basis for the polymorphism of blood group distribution in Yunnan ethnic minority population. At the same time, it provided scientific data for the establishment of rare blood bank of this ethnic group, tissue matching and prevention of immune transfusion reactions. Methods: A blood sample of 5 m L (EDTA anticoagulant) was collected from the Wa ethnic group (150 men and women) with no blood relationship within the third generation of Gengma County in Yunnan Province. The micro-column gel method was used to detect Rh, Duffy and antibody screening, Duffy blood group system Genotyping detection using sequence-specific primer-directed PCR reaction (PCR-SSP), the results obtained with the local Han population for comparative analysis. Results: The Rh D positive rate was 99.0% and the Rh negative rate was 1.0% in 300 cases of Wa nationality. The results of Duffy genotyping showed that Fya gene frequency was 0.994 5, Fyb gene frequency was 0.003 5, Fya-b + Va women had 5 antibody screening test positive, all of the same antibody (E antibody), irregular antibody detection rate of 3.33% of the female population. CONCLUSIONS: The Rh, Duffy blood group antigen distributions and antigen distribution characteristics of the Wa ethnic population in Yunnan are basically consistent with those of the local Han ethnic group, but there are also characteristics of their ethnicity. The detection rate of irregular antibodies among the Va people, especially the women, is higher than that of the Han ethnic group high.
