目的:分析冠状动脉造影(CAG)联合血流储备分数(FFR)在冠心病介入治疗中的应用价值。方法:2014年3月到2015年11月选取来本院就诊的84例冠心病患者,完全随机将其分为两组,每组42例。对观察组患者行FFR测定,在FFR≤0.80的病变位置植入药物洗脱支架(DES),术后在FFR≤0.80处行支架内球囊扩张术;对照组患者仅在病变处行经皮冠状动脉介入治疗(PCI),并于病变位置植入药物洗脱支架。分析比较两组患者的一般情况、植入支架数目、手术时间、造影剂的用量以及术后半年后的主要不良反应的发生概率。结果:对照组患者的一般情况、手术时间以及术后不良反应的发生概率与观察组的差异并无统计学意义(P>0.05)。但是在支架的植入数目和造影剂的使用量上,对照组患者明显多于观察组患者,两者比较差异明显且有统计学意义(P<0.05)。结论:冠状动脉造影联合血流储备分数测定,可以明显减少冠心病患者在介入治疗时植入支架的数量和造影剂的用量,但对手术时间无明显影响,而且随访半年后对增加患者的心脏不良反应概率上无积极作用。
Objective: To analyze the value of coronary angiography (CAG) combined with fractional flow reserve (FFR) in interventional therapy of coronary heart disease. Methods: From March 2014 to November 2015, 84 patients with coronary heart disease selected from our hospital were randomly divided into two groups (n = 42 in each group). The patients in observation group underwent FFR, and the DES was implanted in the lesion with FFR≤0.80. The stent was balloon-expanded at FFR≤0.80 after surgery. The patients in the control group underwent percutaneous coronary intervention Arterial intervention (PCI) and placement of drug-eluting stents at the site of the lesion. The general situation, the number of stent implantation, the operation time, the amount of contrast medium and the incidence of major adverse reactions after six months were analyzed and compared. Results: The general situation, operation time and the incidence of postoperative adverse reactions in the control group were not significantly different from those in the observation group (P> 0.05). However, there were significant differences between the two groups in the number of implants and the amount of contrast medium used in the control group (P <0.05). Conclusion: Coronary angiography combined with the determination of fractional flow reserve can significantly reduce the number of coronary stents and the amount of contrast medium implanted during interventional therapy, but have no significant effect on the operation time, and follow-up of six months to increase the patient’s heart Probability of adverse reactions no positive effect.
