目的:探讨清浊安中汤治疗脾胃湿热证作用机制及脾胃湿热证大鼠模型的建立。方法:50只大鼠随机分为正常对照组、胃炎脾虚证组、胃炎湿热证组、清浊安中汤高、低剂量防治组,不同组用不同药物灌胃,连续20 d。采用“病证结合”方法,以2%水杨酸钠+高脂高糖+人工气候箱复制脾胃湿热证大鼠模型,并用清浊安中汤对脾胃湿热证大鼠模型防治,常规HE胃黏膜病理组织染色、免疫组化(SABC)法检测环氧合酶-2(COX-2)蛋白表达。结果:脾胃湿热证组10只大鼠均有轻-重度炎症,COX-2蛋白表达为0.289±0.036;脾胃湿热证大鼠模型表现出的症状、体征与临床相符,胃黏膜炎症较正常组重而与脾虚证组无差异,COX-2蛋白表达大于脾虚组与正常组(P<0.05),COX-2蛋白表达与炎症程度呈正相关,相关系数为0.662,P<0.01,清浊安中汤防治组对此有改善治疗作用。结论:大鼠模型符合脾胃湿热证属性,调节COX-2蛋白表达可能是清浊安中汤治疗脾胃湿热证作用机制之一。
Objective: To investigate the mechanism of action of Qingzhuo An Zhongtang in treating spleen-stomach damp-heat syndrome and to establish a rat model of spleen-stomach damp-heat syndrome. Methods: Fifty rats were randomly divided into normal control group, gastritis and spleen deficiency syndrome group, gastritis dampness syndrome group and Qingzhuo’anzhongtang high and low dose prevention and treatment groups. The different groups were given gavage with different drugs for 20 consecutive days. Spleen-stomach damp-heat syndrome rat model was duplicated with 2% sodium salicylate + high fat and high glucose + artificial climate chamber by using “combination of disease and syndrome” method. HE staining of gastric mucosa, and the expression of COX-2 protein by immunohistochemistry (SABC) method. Results: The 10 rats in the spleen-stomach damp-heat syndrome group had mild-severe inflammation and the expression of COX-2 protein was 0.289 ± 0.036. The symptoms and signs of rats in the spleen-stomach damp-heat syndrome were consistent with the clinical findings. However, the expression of COX-2 protein in spleen deficiency group was higher than that in spleen deficiency group and normal group (P <0.05). COX-2 protein expression was positively correlated with the degree of inflammation, the correlation coefficient was 0.662, P <0.01. Prevention and treatment group to improve the treatment effect. Conclusion: The rat model accords with the properties of spleen-stomach damp-heat syndrome. Regulating the expression of COX-2 may be one of the mechanisms of the treatment of spleen-stomach damp-heat syndrome by Qingzhuo anzhong decoction.
