目的探讨64层螺旋CT在多发伤早期救治中的作用。方法回顾性分析2006年1月至2007年10月间救治的284例多发伤,男性232例,女性52例;平均年龄34.5岁。头颈伤54例,面部伤35例,胸部伤125例,腹部伤112例,骨盆及四肢伤104例,体表伤254例。合并休克103例。ISS10~54分,平均16.3分。37例因伤情过重直接送入手术室,247例到院后3h内64层螺旋CT检查。结果螺旋CT检查时间8.4min。21例初检时有漏诊。急诊手术154例,其中启动绿色通道74例,非手术治疗130例。治愈272例(95.8%);死亡12例(4.2%)。结论多发伤早期救治中应用64层螺旋CT有助于缩短院内术前时间,提高早期确诊率和救治水平。
Objective To explore the role of 64-slice spiral CT in the early treatment of multiple trauma. Methods A retrospective analysis of 284 cases of multiple traumas treated between January 2006 and October 2007 was performed. There were 232 males and 52 females with a mean age of 34.5 years. Head and neck injuries in 54 cases, 35 cases of facial injuries, chest injuries in 125 cases, 112 cases of abdominal injuries, pelvic and limb injuries in 104 cases, 254 cases of body surface injury. Shock in 103 cases. ISS10 ~ 54 points, an average of 16.3 points. 37 cases were overloaded directly into the operating room due to serious injuries, and 247 cases were examined by 64-slice spiral CT within 3 hours after hospitalization. Results Spiral CT examination time 8.4min. 21 cases missed the first examination. Emergency surgery in 154 cases, of which 74 cases started green channel, non-surgical treatment of 130 cases. 272 cases were cured (95.8%) and 12 died (4.2%). Conclusion The application of 64-slice spiral CT in the early treatment of multiple trauma helps to shorten the preoperative hospital time and improve the early diagnosis and treatment.
