目的:了解孕妇孕期被动吸烟和妊娠期抑郁的流行特征,并分析被动吸烟对妊娠期抑郁的影响。方法:以2013年4月~2014年3月期间分别在同济医院、硚口区妇幼保健院和东西湖人民医院产前检查(产检)和住院分娩的孕妇为研究对象,调查孕妇孕期被动吸烟暴露特征以及孕期抑郁症状的流行状况。采用Cochran-Armitage法分析被动吸烟暴露时间与抑郁症流行率的时间趋势,采用多因素Logistic回归分析被动吸烟对抑郁症的危险。结果:共调查孕妇900名,其中有效问卷791份,有效应答率为87.89%。孕期被动吸烟的暴露率为34.26%,妊娠期抑郁的流行率为17.07%,随着暴露时间增加,抑郁症的流行率呈增加趋势。孕晚期抑郁的发生率(77.04%)高于孕早中期(22.96%),差异有统计学意义(P<0.001)。多因素Logistic回归分析显示,每日被动吸烟暴露时间超过60 min可显著增加妊娠期抑郁的发生风险(OR=4.89,95%CI:1.81~13.20)。结论:孕妇孕期被动吸烟的暴露较高,是孕妇抑郁发生的危险因素。应加强宣传教育,降低被动吸烟率,减少孕期抑郁症发生。
Objective: To understand the prevalence of passive smoking during pregnancy and gestational depression during pregnancy, and to analyze the influence of passive smoking on the depression during pregnancy. Methods: From April 2013 to March 2014, pregnant women at Tongji Hospital, Qiaokou Maternal and Child Health Hospital and East-West Lake People’s Hospital during antenatal examination (check-up) and hospital delivery were investigated. Features and prevalence of depressive symptoms during pregnancy. The time trend of passive smoking exposure time and prevalence of depression was analyzed by Cochran-Armitage method. Multivariate logistic regression was used to analyze the risk of passive smoking on depression. Results: A total of 900 pregnant women were surveyed, of which 791 were valid questionnaires. The effective response rate was 87.89%. The rate of passive smoking during pregnancy was 34.26% and the prevalence of depression during pregnancy was 17.07%. With the increase of exposure time, the prevalence of depression increased. The incidence of depression in late pregnancy (77.04%) was higher than that in early pregnancy (22.96%), the difference was statistically significant (P <0.001). Multivariate Logistic regression analysis showed that the daily passive smoking exposure time of more than 60 min significantly increased the risk of depression in pregnancy (OR = 4.89, 95% CI: 1.81 ~ 13.20). Conclusion: The high exposure of pregnant women during passive smoking during pregnancy is a risk factor for depression in pregnant women. Should strengthen publicity and education, reduce passive smoking rates, reduce depression during pregnancy.
