目的探究雷公藤多苷联合环磷酰胺对类风湿关节炎(RA)患者血清血管内皮生长因子(VEGF)、血管内皮细胞生长因子受体2(VEGFR2)表达水平的影响。方法选取新乡市医学院第三附属医院2015年3月—2016年4月74例类风湿关节炎患者,采用随机数表法分为对照组和研究组,每组37例。对照组采用环磷酰胺,研究组采用环磷酰胺+雷公藤多苷,两组均持续治疗2个月。统计两组临床疗效、不良反应发生率,对比治疗前后两组血清VEGF、VEGFR2表达水平变化。结果研究组治疗有效率(91.89%)高于对照组(72.97%),差异有统计学意义(P<0.05);治疗前两组VEGF及VEGFR2水平比较差异无统计学意义(P>0.05),治疗后研究组各指标水平低于对照组,差异有统计学意义(P<0.05);研究组不良反应发生率(16.21%)与对照组(21.63%)对比,差异无统计学意义(P>0.05)。结论联合应用雷公藤多苷与环磷酰胺治疗类风湿关节炎效果显著,可有效降低血清VEGF、VEGFR2表达水平,提高治疗效果,且安全性较高。
Objective To investigate the effects of tripterygium glycosides combined with cyclophosphamide on the expression of vascular endothelial growth factor (VEGF) and vascular endothelial growth factor receptor 2 (VEGFR2) in patients with rheumatoid arthritis (RA). Methods 74 patients with rheumatoid arthritis were selected from March 2015 to April 2016 in the Third Affiliated Hospital of Xinxiang Medical College, and divided into control group and study group with 37 cases in each group. In the control group, cyclophosphamide was used. In the study group, cyclophosphamide and tripterygium glycosides were used. Both groups were treated for 2 months. The clinical efficacy and incidence of adverse reactions were compared between the two groups, and the changes of serum VEGF and VEGFR2 levels were compared before and after treatment. Results The effective rate of the treatment group (91.89%) was significantly higher than that of the control group (72.97%) (P <0.05). There was no significant difference in the levels of VEGF and VEGFR2 between the two groups before treatment (P> 0.05) After treatment, the level of each index in the study group was lower than that in the control group (P <0.05). There was no significant difference in the incidence of adverse reactions between the study group (16.21%) and the control group (21.63%) (P> 0.05). Conclusions The combined application of tripterygium glycosides and cyclophosphamide in the treatment of rheumatoid arthritis is significant, which can effectively reduce the expression of VEGF and VEGFR2, improve the therapeutic effect and have higher safety.
