【目的】比较采取飞秒激光制瓣、moria旋转刀制瓣、moria平推刀制瓣前弹力层下角膜磨镶术对角膜形态和生物力学指标的影响,为选择合适的制瓣方式提供参考。【方法】选取2012年3月至2015年2月来本科行准分子激光原位角膜磨镶术治疗患者1200例(2400眼),采用随机数表法分为激光组、旋转刀组和平推刀组,对比分析三组术后1个月、3月个各方向角膜瓣厚度;术前及术后1个月、3个月角膜阻力因子、角膜滞后量的差异。【结果】激光组术后1个月、3个月角膜瓣顶点处角膜瓣厚度分别为(111±7)μm、(111±7)μm ,差异无统计学意义(P >00.5);旋转刀组术后1个月、3个月角膜瓣顶点处角膜瓣厚度分别为(118±8)μm、(117±8)μm ,差异无统计学意义(P >00.5);平推刀组术后1个月、3个月角膜瓣顶点处角膜瓣厚度分别为(126±9)μm、(127±9)μm ,差异无统计学意义(P >00.5)。三组术前角膜阻力因子差异无统计学意义(P>00.5);角膜阻力因子术后同时期比较,激光组>旋转刀组>平推刀组,差异均具有统计学意义( P <00.5)。三组术前角膜滞后量差异无统计学意义( P >00.5);三组术后角膜滞后量同时期比较,激光组>旋转刀组>平推刀组,差异均具有统计学意义( P <00.5)。【结论】前弹力层下角膜磨镶术飞秒激光制瓣较 moria旋转刀、moria平推刀制瓣具有厚度更均匀、偏差更小、生物力学指标参数更高等优点,值得临床借鉴推广。“,”Objective]To compare and analyze the influence of femtosecond laser flap creation ,moria ro‐tating knife flap creation ,and moria flat push knife on corneal flap morphology and corneal flap biomechanics of SBK ,as well as to provide reference to flap creation .[Methods]The 1200 cases of patients (2400 eyes) who had received SBK treatment between March 2013 to February 2015 in our hospital were selected as study ob‐jects .They were divided using the random number table method into three groups :a laser group ,a rotating knife group ,and moria flat push knife group .Differences among the three groups in corneal flap thickness 1 month and 3 months postoperative ,as well as corneal hysteresis and corneal resistance 1 and 3 months before and after the operation were measured and analyzed .Evaluation provided information on the clinical value of femtosecond laser flap creation .[Results]The corneal flap thickness 1 month and 3 months postoperative in the laser group were(111 ± 7)μm and(111 ± 7)μm ;there was no significant difference ( P >0 0.5) .The corne‐al flap thickness 1 month and 3 months postoperative in the moria rotating knife group were (118 ± 8)μm and (117 ± 8)μm ;there was no significant difference ( P >0 0.5) .The corneal flap thickness 1month and 3months postoperative in the moria flat push group were(126 ± 9)μm and(127 ± 9)μm ;there was no significant differ‐ence ( P >0 0.5) .There were no significant differences among the three groups in the corneal resistance factor before operation ( P >0 0.5) .Comparison of corneal resistance post -operation in the same period :laser group>rotating knife group> flat push knife group ,and the difference was statistically significant ( P 0 0.5) .Corneal hysteresis postoperation among the three groups :laser group>rotating knife group>flat push knife group ,the difference was statistically significant ( P <0 0.5) .[Conclusion]Compared to the moria rota‐ting knife flap creation and moria flat push knife ,the femtosecond laser flap creation provided more uniform thickness ,smaller deviation ,and higher biomechanics indicator ,it is worthy of reference promotion .
