目的分析儿童甲状腺病变的核医学~(99)mTcO_4~-甲状腺显像及CT的表现特征,结合临床及术后病理,总结甲状腺的病变及CT表现,并探讨其诊断价值。方法搜集本院2009年12月-2016年5月48例儿童甲状腺病变(男14例,女34例,中位年龄11.5岁),且行~(99)mTcO_4~-甲状腺显像和/或CT检查患者的病例资料,其中19例甲状腺肿块病例均经手术证实,余29例结合临床及实验室检查诊断。将影像诊断(~(99)mTcO_4~-甲状腺显像、CT)与术后病理结果及临床诊断进行对照分析。结果 19例甲状腺肿块中包括腺瘤8例、腺癌5例、结节性甲状腺肿3例、甲状腺脓肿2例、腺旁错构瘤1例。9例异位甲状腺中1例为舌骨水平上下3个结节状改变、1例为舌根及颈前双重异位、余5例位于舌根部(2例为舌根区单个异位改变)。8例正常位置未见明显放射性摄取,1例可见增大的甲状腺,且摄取明显增多。甲状腺炎症根据病程变化有不同程度的摄取增加或减少。结论~(99)mTcO_4~-甲状腺显像及CT在诊断甲状腺病变中有重要辅助作用。
Objective To analyze the clinical features of ~ (99) mTcO_4 ~ - thyroid imaging and CT in children with thyroid lesions, and to summarize the clinical and postoperative pathology, and to summarize the thyroid lesions and CT findings and to explore its diagnostic value. Methods Forty-eight children with thyroid lesions (14 males and 34 females, with a median age of 11.5 years) from December 2009 to May 2016 were enrolled in this study. 99TcO_4 ~ thyroid imaging and / or CT were performed Patient’s case information, including 19 cases of thyroid mass were confirmed by surgery, more than 29 cases combined with clinical and laboratory diagnosis. Imaging diagnosis (~ (99) mTcO_4 ~ - thyroid imaging, CT) and postoperative pathological results and clinical diagnosis were analyzed. Results 19 cases of thyroid mass included 8 cases of adenoma, 5 cases of adenocarcinoma, 3 cases of nodular goiter, 2 cases of thyroid abscess and 1 case of parathyroid hamartoma. One of the nine cases of ectopic thyroid gland had three nodular changes at the level of the hyoid bone. One case had double heterosis of tongue base and anterior cervical neck, and the other five cases located in the base of tongue (two cases were single ectopic change in the base of tongue). No significant radioactive uptake was seen in 8 patients in the normal position, and an increased thyroid was seen in 1 patient and the uptake was significantly increased. Thyroid inflammation according to the course of change have different degrees of increase or decrease. Conclusion ~ (99) mTcO_4 ~ - thyroid imaging and CT in the diagnosis of thyroid disease has an important supporting role.
