目的:分析五官科患者围术期抗菌药物使用合理性,并寻求相关有效对策及建议,指导五官科患者围术期抗菌药物的使用。方法:采用随机抽样法对南浔区人民医院五官科于2014年6月~2015年7月收治的120例患者病历进行收集整理,并统计五官科患者围术期各抗菌药物使用情况,并依据围术期抗菌药物使用指南对五官科患者围术期抗菌药物使用情况进行合理性分析。结果:在五官科患者围术期抗菌药物使用中各有差异,主要包括抗菌药物的种类、频次及给药时间有差异,且各抗菌药物总使用频次为423例次,主要使用的抗菌药物有青霉素类143例次、硝基咪唑类126例次及头孢菌素类96例次等;单用一种抗菌药物有29例,两种联合使用的有68例,三种及以上联合使用的有23例;此次病历中依据各手术缓急病情统计为41例手术前24小时给药、32例手术前2小时给药、12例手术前0.5小时给药、13例手术后0.5小时给药及22例手术后6小时给药。研究分析此次病历统计五官科患者围术期抗菌药物合理性用药108例,12例有无指征而用药、用药时常延长及联合用药不当等。结论:依据统计调查结果,五官科患者围术期抗菌药物使用方面仍有待进一步研究提高用药科学性,减少抗菌药物的不合理使用,确保临床用药安全性。
Objective: To analyze the rational use of perioperative antibiotics in patients with ENT and to seek effective countermeasures and suggestions to guide the use of perioperative antibiotics in patients with ENT. Methods: A total of 120 patients in Nan Nan District People’s Hospital were collected from June 2014 to July 2015 by random sampling method. The usage of antibacterials during the perioperative period was analyzed. The use of antimicrobial guidelines during the operation of patients with ENT perioperative rational use of antimicrobial agents. Results: There were differences in the use of antimicrobial agents during the perioperative period in patients with ENT, including the types, frequency and administration time of antimicrobial agents. The total frequency of antimicrobial agents used was 423 cases. The main antimicrobial agents used were 143 cases of penicillin, 126 cases of nitroimidazoles and 96 cases of cephalosporins second class; single use of an antibacterial drug in 29 cases, the two used in combination of 68 cases, three or more of the joint use of 23 cases; the medical records based on the critical condition of each surgery, 41 cases were administered 24 hours before surgery, 32 cases were given 2 hours before surgery, 12 cases were given 0.5 hours before surgery and 13 cases were given 0.5 hours after surgery and Twenty-two patients were given 6 hours after surgery. Analysis of the medical records of patients with ENT patients during perioperative rational use of antibiotics in 108 cases, 12 cases were indications and medication, medication is often extended and improper combination medication. Conclusions: According to the results of the survey, the use of perioperative antibiotics in patients with ENT remains to be further studied to improve the scientific use of medicines, reduce the irrational use of antimicrobial agents and ensure the safety of clinical medication.
