目的了解一般人群成年男性非酒精性脂肪肝(NAFLD)的患病率,分析其危险因素。方法以参加防城港市第一人民医院体检的男性人群作为研究对象,内容涉及问卷、一般体格检查、血生化检测及肝脏实时超声检查,筛选出3 238例有完整调查问卷和检查资料的成年男性进行研究。结果 3 238例中检出NAFLD832例,患病率为25.69%。男性50岁以前NAFLD患病率均随年龄增长而增加,而5岁以后逐渐下降。NAFLD组年龄、腰围、体重指数(BMI)、腰臀比、收缩压、舒张压、空腹血糖(FPG)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、谷丙转氨酶(ALT)、尿酸(UA)以及超重、肥胖、高TC血症、高TG血症、空腹血糖受损、糖尿病、高血压症、高UA血症和肝功能异常的检出率均明显高于对照组,高密度脂蛋白胆固醇(HDL-C)低于对照组。Logistic回归分析显示TG、BMI、腰围、FPG、HDL-C、ALT、UA与男性NAFLD的发生密切相关(P<0.05)。结论防城港市成年男性NAFLD患病率高,TG、BMI、腰围、FPG是男性患NAFLD的危险因素,HDL-C是其保护因素。
Objective To investigate the prevalence of non-alcoholic fatty liver disease (NAFLD) in the general population and to analyze its risk factors. Methods The male population who participated in the physical examination of the First People’s Hospital of Fangchenggang City was investigated. The questionnaire, general physical examination, blood biochemistry test and real-time liver ultrasound examination were performed. A total of 3 238 adult males with complete questionnaire and examination data were screened out the study. Results A total of 832 NAFLD cases were detected in 3 238 cases, with a prevalence of 25.69%. The prevalence of NAFLD in males before age 50 increased with age, but decreased gradually after 5 years of age. Waist circumference, body mass index (BMI), waist-hip ratio, systolic blood pressure, diastolic blood pressure, fasting blood glucose (FPG), total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol ), Alanine aminotransferase (ALT), uric acid (UA) and detection of overweight, obesity, hypercholesteremia, hypercholesteremia, impaired fasting glucose, diabetes, hypertension, hyperuricaemia and liver dysfunction Rate were significantly higher than the control group, high-density lipoprotein cholesterol (HDL-C) was lower than the control group. Logistic regression analysis showed that TG, BMI, waist circumference, FPG, HDL-C, ALT and UA were closely related to the occurrence of NAFLD in men (P <0.05). Conclusion The prevalence of NAFLD in adult male is higher in Fangchenggang city. TG, BMI, waist circumference and FPG are the risk factors of NAFLD in male, and HDL-C is the protective factor.
