目的对糖尿病、颈动脉斑块以及脑梗死三者之间的相关性进行分析和研究。方法此次临床研究主要以本院2010年2月至2012年12月收治的230例缺血性脑血管病患者为研究对象,经头颅MRI检测,患者未出现新发脑梗死。结合颈动脉超声检查显示动脉内膜中层厚度(IMT),将其分为有斑块形成组以及无斑块形成组,并对患者的生化指标、性别、年龄、有无高血压及糖尿病进行记录和分析。使用二分类法对糖尿病、颈动脉斑块以及脑梗死三者的阳性率进行比较,并对三者的相关性进行分析。结果脑梗死组的斑块阳性率高于无脑梗死组,斑块组的脑梗死发生率高于无斑块组,对其进行Logistic回归分析,校正前后得出OR值分别为5.017、4.457,95%CI为2.692~9.352、2.059~9.645,P<0.05;斑块组的糖尿病阳性率高于无斑块组,糖尿病组的颈动脉斑块阳性率高于无糖尿病组,对其进行Logistic回归分析,校正前后得出OR值为4.008、4.270,95%CI为1.852~8.670、1.563~11.660,P<0.05;脑梗死组的糖尿病阳性率高于无脑梗死组,糖尿病组的脑梗死发生率高于无糖尿病组,对其进行Logistic回归分析,得出OR值为2.254,95%CI为1.240~4.093,P<0.05,引入危险因素后,OR值为2.103,95%CI为1.064~4.155,P<0.05,引入颈动脉斑块后,OR值为1.694,95%CI为0.831~3.450,P>0.05。结论糖尿病能够对颈动脉斑块的形成产生影响,进而对脑梗死的发生产生影响。
Objective To analyze the relationship between diabetes mellitus, carotid artery plaque and cerebral infarction. Methods The clinical study of 230 patients with ischemic cerebrovascular disease admitted to our hospital from February 2010 to December 2012 was selected as the research object. No new-onset cerebral infarction occurred in the patients after cranial MRI examination.
