目的探讨盆底肌锻炼配合电刺激治疗产后压力性尿失禁(SUI)的临床效果,分析不同盆底肌锻炼配合电刺激开始时期对SUI的影响。方法选取2013年8月-2015年1月在该院分娩后行盆底肌功能恢复治疗的SUI患者122例为联合组,根据给予盆底肌锻炼配合电刺激治疗方式开始时间分为短期组、中期组及长期组。另选取同期来我院复查的SUI患者41例为对照组。比较两组患者一般信息、尿失禁临床症状及严重分度、盆底肌力电生理检查等指标的差异,对比研究盆底肌锻炼配合电刺激治疗对SUI的影响。结果联合组患者治疗后尿失禁症状明显减少,盆底Ⅰ类和Ⅱ类肌纤维肌力均有显著回升,与治疗前比较,差异有统计学意义(P<0.05)。治疗前联合组与对照组、对照组治疗前后各指标比较,差异无统计学意义(P>0.05),治疗后两组各指标比较,差异有统计学意义(P<0.05)。开始盆底肌锻炼配合电刺激治疗的时间不同对患者各指标无明显影响,差异无统计学意义(P>0.05)。结论盆底肌的恢复能够明显降低SUI阳性率,且经长期的功能恢复治疗并不会增加患者不良反应,并可提高产后孕妇的生活质量。
Objective To investigate the clinical effect of pelvic floor muscle exercise combined with electrical stimulation in the treatment of postpartum stress urinary incontinence (SUI), and to analyze the effect of different pelvic floor exercises on SUI during the beginning of electrical stimulation. Methods A total of 122 SUI patients who underwent pelvic floor muscle function recovery after delivery from August 2013 to January 2015 in our hospital were selected as the combined group. According to the onset time of pelvic floor muscle stimulation and electrical stimulation, they were divided into short-term group, Mid-term group and long-term group. In addition, 41 patients with SUI in our hospital during the same period were selected as the control group. The general information of two groups of patients, urinary incontinence clinical symptoms and severe degree, pelvic floor electrophysiological examination and other indicators of the differences were compared pelvic floor exercises with electrical stimulation on SUI. Results The symptoms of urinary incontinence in the combined group were significantly decreased. The muscle strength of type Ⅰ and Ⅱ myofibers in the pelvic floor were significantly increased. Compared with those before treatment, the difference was statistically significant (P <0.05). There was no significant difference between the two groups before and after treatment (P> 0.05). The difference between the two groups after treatment was statistically significant (P <0.05). The beginning of pelvic floor muscle exercise with electrical stimulation of different treatment time had no significant effect on the various indicators of patients, the difference was not statistically significant (P> 0.05). Conclusion Pelvic floor muscle recovery can significantly reduce the SUI positive rate, and long-term functional recovery does not increase adverse reactions in patients and improve the quality of life of postpartum pregnant women.
