目的:探讨CT对COPD患者颅内血管改变的诊断意义。方法:采用MSCT行脑CTP检查,对生成的CBF、CBV、MTT3种参数与对照组进行对比;并行脑CTA检查,观察脑内大血管分支狭窄及闭塞情况,并分析该情况的高发部位。结果:在研究中我们发现,Ⅲ级、Ⅳ级COPD患者CBF、CBV、MTT3种参数与正常组相比,有明显差异(P<0.05);脑CTA检查发现随着COPD程度的加重,颅内动脉狭窄发生率越高,并好发于大脑中动脉、颈内动脉、大脑后动脉、基底动脉、大脑前动脉。结论:COPD患者可合并颅内血管及血液灌注的改变,CT检查能够早期快速、准确反映颅内血管病变的程度及部位,对该病的预防和治疗有重要意义。
Objective: To investigate the significance of CT in the diagnosis of intracranial vascular changes in COPD patients. Methods: The cerebral CT angiography was performed by MSCT. The CBF, CBV and MTT parameters were compared with those of the control group. Brain CTA examination was performed to observe the stenosis and occlusion of the large blood vessel branches in the brain and to analyze the high incidence of this condition. Results: In the study, we found that the parameters of CBF, CBV and MTT in grade Ⅲ and Ⅳ COPD patients were significantly different from those in normal group (P <0.05). The brain CTA examination showed that as the degree of COPD increased, The higher the incidence of arterial stenosis, and occur in the middle cerebral artery, internal carotid artery, posterior cerebral artery, basilar artery, anterior cerebral artery. Conclusion: COPD patients with intracranial blood vessels and blood perfusion changes, CT examination can quickly and accurately reflect the degree and location of intracranial vascular lesions, the prevention and treatment of the disease is of great significance.
