目的探讨体重指数(BMI)对绝经前后Luminal型乳腺癌预后的影响。方法回顾性收集四川省肿瘤医院2005-2010年收治的298例女性原发Luminal型乳腺癌病例资料并进行随访,中位随访时间为39个月。将病例根据绝经状态分层,采用COX比例风险模型分析不同BMI(BMI<23.0 kg/m2和BMI≥23.0 kg/m2)对乳腺癌病例无病生存期(DFS)的影响。结果绝经前和绝经后Luminal型乳腺癌患者的平均BMI分别为(22.77±2.67)kg/m2和(24.47±3.63)kg/m2,差异有统计学意义(P<0.001),BMI<23.0 kg/m2和BMI≥23.0 kg/m2的绝经前Luminal型乳腺癌患者的5年无病生存率分别为78.9%和60.7%(P=0.036),BMI≥23 kg/m2可显著增加绝经前Luminal型乳腺癌复发转移的风险(调整HR=2.451,95%CI:1.100~5.459),BMI对绝经后Luminal型乳腺癌DFS的影响无统计学意义。结论高BMI是影响绝经前Luminal型乳腺癌预后的不良因素。
Objective To investigate the effect of body mass index (BMI) on the prognosis of Luminal-type breast cancer before and after menopause. Methods The data of 298 cases of primary Luminal breast cancer in Sichuan Cancer Hospital from 2005 to 2010 were retrospectively collected. The median follow-up time was 39 months. The cases were stratified according to menopausal status and the COX proportional hazards model was used to analyze the effect of different BMIs (BMI <23.0 kg / m2 and BMI ≥ 23.0 kg / m2) on disease-free survival (DFS) in patients with breast cancer. Results The mean BMI of premenopausal and postmenopausal women with Luminal breast cancer were (22.77 ± 2.67) kg / m2 and (24.47 ± 3.63) kg / m2, respectively, with statistical significance (P <0.001) m2 and BMI≥23.0 kg / m2, the 5-year disease-free survival rates of premenopausal women with Luminal breast cancer were 78.9% and 60.7%, respectively (P = 0.036). BMI≥23 kg / m2 could significantly increase the premenopausal Luminal breast The risk of cancer recurrence and metastasis (adjusted HR = 2.451, 95% CI: 1.100 ~ 5.459), BMI had no significant effect on DFS of Luminal breast cancer after menopause. Conclusion High BMI is an adverse factor affecting the prognosis of premenopausal women with Luminal breast cancer.
